About The Position

This role involves providing specialized pharmaceutical care within Infectious Disease, focusing on antimicrobial stewardship, pharmacotherapy, and patient safety. The Advanced Pharmacist will actively monitor trends in antimicrobial utilization and resistance, lead infectious disease section meetings, and participate in infection prevention initiatives. They will offer consultative services in infectious diseases pharmacotherapy and pharmacokinetics, evaluate drug therapies, and research and interpret data from drug information resources. A key responsibility includes building the annual site-specific antibiogram and participating in quality improvement projects. The role also involves conducting medication use evaluations and research projects, potentially leading to publications or presentations, and precepting pharmacy residents and students.

Responsibilities

  • Reviewing patient medication regimens and providing recommendations to optimize therapy based on clinical guidelines and best practices.
  • Conducting medication reconciliation to ensure accuracy and safety during transitions of care.
  • Educating patients and caregivers on proper medication use, side effects, and adherence strategies.
  • Collaborating with healthcare providers to develop and implement patient-specific care plans.
  • Monitoring patient outcomes and adjusting therapy as necessary to achieve therapeutic goals.
  • Participating in interdisciplinary rounds and providing pharmacotherapy recommendations.
  • Providing drug information to healthcare professionals and patients as needed.
  • Contributing to quality improvement initiatives related to medication use and patient safety.
  • Documenting interventions, clinical assessments, and recommendations in the electronic health record.
  • Staying current with developments in pharmacotherapy and evidence-based medicine.
  • Serving as a preceptor and mentor for pharmacists, pharmacy residents, and students as needed.
  • Actively viewing trends in antimicrobial utilization and antimicrobial resistance patterns.
  • Leading infectious disease section meetings with the Infectious Disease section medical Chief.
  • Participating in site and system-wide Infection Prevention and Antimicrobial Stewardship meetings.
  • Providing consultative service in infectious diseases pharmacotherapy and pharmacokinetics, examining the pharmacoeconomic impact of providing care to patients.
  • Participating in drug therapy evaluations both prospectively and retrospectively.
  • Demonstrating knowledge of various drug information resources and being able to research and interpret data and information.
  • Building annual site-specific antibiogram.
  • Participating in quality, safety, and other project work related to infectious diseases.
  • Conducting medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization with publication and/or presentation at local and/or national meetings.
  • Precepting pharmacy residents and pharmacy students.
