About The Position

HarmonyCares is seeking an Advance Practice Provider (NP/PA) to join their team. This role focuses on chronic condition management for a geriatric panel, allowing for 30-45 minute patient visits. The position offers flexibility with either a 32hr/week (4-day) or 40hr/week (5-day) option, operating Monday through Friday with no weekends or evenings. Requirements

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record

Nice To Haves

• Geriatric training/experience preferred
• Directly related experience in geriatrics and primary care

Responsibilities

• Chronic condition management of your own geriatric panel
• Spend 30-45min with your patients
• Provide primary care to high acuity patients in their home Monday - Friday
