Requirements

  • Master’s degree in nursing or related science field required
  • Active and unrestricted NP license required
  • 2+ years of strong leadership and management experience in a clinical setting
  • Experience with Medicare/Medicaid is a must
  • 2+ years of clinical experience treating complex chronic conditions and high-needs patients
  • Excellent verbal, written, presentation, and group facilitation skills
  • Strong critical analytic, thinking and communication skills necessary to evaluate and respond to team progress, anticipate needs and other related team dynamics
  • Excellent computer skills required particularly Microsoft applications including Word, Excel, PowerPoint, and Outlook
  • Willingness to explore and learn new or unfamiliar applications that may benefit the clinical team
  • Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced organization.

Nice To Haves

  • Community-based care experience preferred
  • Medicare and LTSS experience preferred

Responsibilities

  • Training and day-to-day management of Nurse Practitioners (NPs), Physician Assistants (PAs), and RN’s, ensuring effective collaboration, proper clinical management, documentation of visits, and high-quality patient care delivery.
  • Team leadership, performance monitoring, and overseeing an interdisciplinary team to meet organizational goals, improve member outcomes, and provide exceptional patient care.
  • Managing processes to ensure members and their families receive timely and effective navigation support, both telephonically and in the field.
  • Mentoring clinical team members, providing clinical guidance, regular performance evaluations, constructive feedback, and supporting professional development.
  • Implementing evidence-based practices and clinical protocols.
  • Monitoring and evaluating patient care delivery to maintain high standards of quality and safety.
  • Ensuring staff schedules optimize productivity, meeting patient care needs, maintaining high standards of care and safety in compliance with regulatory requirements.
  • Addressing and resolving patient or staff concerns promptly and professionally.
  • Coordinating orientation and continuing education for clinical staff, promoting opportunities for professional growth.
  • Serving as a liaison between clinical staff and administrative leadership.
  • Participating in interdisciplinary team meetings to enhance patient care coordination.
  • Ensuring all staff adhere to regulatory standards, including HIPAA and OSHA requirements.
  • Maintaining accurate documentation of clinical activities, training, and compliance records.
  • Conducting patient visits in-person in the patients' homes and/or via telehealth maintaining a patient panel equivalent to 0.6 FTE.
  • Collaborating with business partners such as Home Health and Hospice agencies, assisted living facilities, Primary Care providers, Specialists, hospital staff and other preferred partners.
  • Collaborating with multiple care disciplines such as RN's, and cross-functional team-members to provide high-quality care.
  • Participating in after-hours on-call coverage rotation and rotating evening and weekend patient care coverage.
