About The Position

The Advanced Practice Nurse (APN/APRN) provides comprehensive, patient-centered medical care, including assessment, diagnosis, treatment, and management of acute and chronic conditions. Working collaboratively with physicians and interdisciplinary healthcare teams, the APN delivers high-quality, evidence-based care in accordance with scope of practice, regulatory requirements, and organizational policies.

Requirements

  • Master’s degree or Doctorate in Nursing from an accredited Advanced Practice program required.
  • Active, unrestricted Advanced Practice Nurse license in the state of practice.
  • National certification as a Nurse Practitioner (ANCC or AANP) required.
  • DEA registration and controlled substance authority (as applicable).
  • BLS certification required.
  • Strong clinical assessment, diagnostic, and decision-making skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and as part of an interdisciplinary team.
  • Proficiency with Electronic Health Records (EHR) systems.
  • Commitment to ethical practice, patient-centered care, and professionalism.

Nice To Haves

  • ACLS preferred.
  • Minimum of 1–3 years of clinical experience as an Advanced Practice Nurse preferred.
  • Experience in primary care, urgent care, behavioral health, or correctional health preferred.

Responsibilities

  • Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic evaluations.
  • Diagnose and manage acute and chronic medical conditions within scope of practice.
  • Prescribe medications, treatments, and therapeutic interventions in accordance with state regulations and organizational policy.
  • Order, interpret, and evaluate diagnostic tests and laboratory results.
  • Develop, implement, and evaluate individualized treatment plans.
  • Provide patient education, health promotion, disease prevention, and counseling services.
  • Collaborate with physicians, nurses, behavioral health providers, and other healthcare professionals to coordinate care.
  • Document all patient encounters accurately and timely within the Electronic Health Record (EHR).
  • Participate in on-call coverage, clinical rotations, or shift coverage as assigned.
  • Adhere to infection control standards, medication management policies, and controlled substance regulations.
  • Participate in quality improvement, utilization review, and performance improvement initiatives.
  • Ensure compliance with federal, state, and facility regulations.
  • Provides a positive and professional representation of CFG Health Systems, LLC.
  • Promotes a culture of safety for patients and employees through proper identification, reporting, documentation, and prevention.
  • Maintains competency and knowledge of current standards of practice, trends, and developments related to the scope of practice.
  • Adheres to infection control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
  • Participates in ongoing quality improvement activities.
  • Completes required orientation and training as directed by the facility.
  • Follows facility policies, OSHA safety rules, and procedures while on assignment.
  • Follows facility and CFG Health Systems Occurrence Protocol.
  • Upholds HIPAA and PREA regulations.
  • Demonstrates punctuality and dependability for assigned and confirmed shifts.
  • Maintains required security clearance for the assigned facility.
