About The Position

This person is responsible for the smooth running of a caseload of individuals in Children Habilitation Residential Program/Host Home and Family Care Giver programs which includes financial management and overseeing program compliance. This person provides leadership, assistance, supervision, training, and support to Independent Contractors and Individuals served. It is expected that this person is familiar with governing rules and regulations for the programs and that the program maintains compliance with these rules and regulations.

Requirements

  • Bachelor's degree and 3 years of human services experience.
  • Valid Colorado Driver's License (or ability to obtain within 30 days of hire) with 5 years of driving experience.
  • Ability to read, write, and speak the English language for required paperwork, testing, and professional communication.
  • Ability to operate a computer utilizing Microsoft Office.
  • Knowledge and experience with the electronic record management system Therap.
  • Satisfactory background check.
  • Must be able to resolve problems, handle conflict, and make effective decisions under pressure while dealing with a variety of emotions and frustrations.
  • Requires scheduling flexibility.
  • Ability to work as part of a team.
  • Ability to communicate effectively with respect and tact in order to give, receive, and analyze information, formulate plans, prepare materials.
  • Ability to supervise and effectively manage Independent Contractors.
  • Ability to organize and time manage all required tasks.

Responsibilities

  • Ensure compliance with all applicable regulations, including CDPHE, HCPF, and the Final Settings Rule.
  • Experience supervising or coordinating Independent Contractors or similar personnel.
  • Oversee documentation, incident reporting, and program quality.
  • Provide leadership, supervision, and support to Independent Contractors.
  • Oversee services and programming for individuals in the Children Habilitation Residential Program/Host Home (CHRP-HH) and Family Caregiver (FCG) programs.
  • Respond effectively to emergencies and participate in the on-call rotation.
  • Uphold the mission, values, policies and procedures of RiseUP.
  • Uphold principles of the trauma informed principles of RiseUP.
  • Participate in training and events that specifically address the intersection of trauma with culture, history, race, gender, location, and language.
  • Oversee documentation is in compliance including follow-up on incident reports.
  • Identify support for persons in services and develop a plan for addressing these needs.
  • Oversight to Independent Contractors.
  • Provide direction, support, and role modeling to Independent Contractors.
  • Support persons in services with Informed Decision Making.
  • Oversee Person Centered Services.
  • Ensure accountability of all program money.
  • Maintain frequent and professional communication with supervisors, family members, Host Home Providers (HHPs), FCGs, medical professionals, staff members, and other professionals or Interdisciplinary Team Members (IDT).
  • Handle emergency situations effectively while following established procedures.
  • Participate in the organization’s Quality Improvement process.
  • Provide feedback to Leadership when there are concerns about policies and procedures.
  • Responsible for supporting and maintaining a physically and psychologically safe environment.
  • Participate in the on-call rotation.
  • Perform other duties as assigned by supervisor.
