Provides assessment and case management services to elderly and disabled adults who are at risk of neglect, abuse, or exploitation. Requirements

  • A bachelor’s degree from an accredited institution with a major in a human behavior science field, OR a degree with 30 semester hours, or 45 quarter hours, of qualified work in development in human behavioral sciences or health care related courses, such as, social work, sociology, psychology, psychiatry, gerontology, nursing special education, family intervention techniques, diagnostic measures, therapeutic techniques, guidance and counseling, criminal justice, or other human behavioral sciences, or a medical field relevant to the APS program and/or at-risk adults.
  • One year of professional casework in a public or private social services agency completed after the degree is obtained.
  • Must have valid driver's license.
  • If you are from out of state, you must obtain a valid Colorado driver's license within 30 days of hire date.
  • Must not have any major violations, including DUI, DWI, and/or DWAI conviction(s) in the past three years.

Nice To Haves

  • Experience working with adults is preferred.
  • Bilingual (English/Spanish) is desired.

Responsibilities

  • Provides one-on-one assessments of at-risk adults who are unable to protect their own interests based on physical appearance and hygiene; medical, nutritional, and environmental needs; competency and ability to understand self-determination and the result of the situation that is placing him/her at risk; activities of daily living; support systems; and ability to manage financial affairs.
  • Seeks court action to obtain protective services and/or potential guardianships in emergency situations.
  • Works closely with law enforcement agencies and district attorney’s office in investigations of abuse, neglect, and exploitation of at-risk elders.
  • Works collaboratively with multiple community agencies to assess, investigate, and implement services for at-risk adults.
  • Performs case planning for individuals who require ongoing protective services which may include referral for services to reduce harm and risk.
  • Provides coordination of assessments and/or ongoing service delivery with appropriate collateral agencies.
  • May attend inter-disciplinary team meetings regarding client care, advocacy, and service coordination; attends conferences and meetings of various types to accrue mandatory continuing education hours.
  • Provides outreach, information and education to the community and organizations regarding adult protection services.
  • Documents assessments and actions taken throughout all phases of the case.
  • Oral and written communication must be clear.
  • Performs other related duties as assigned.
