About The Position

Cayuga Counseling Services, Inc. is seeking a Full-Time Adult Intensive Case Manager in Cayuga County to provide intensive services to adult clients that are in the Cayuga County Jail and assist with their transition back into the community. The role aims to restore, rehabilitate, and support the individual client as an active and productive member of their family, community, and/or culture with the least amount of ongoing professional intervention. This includes assisting the individual in acquiring, retaining, and improving skills such as communication, self-help, domestic, socialization, and self-care to successfully reside in a home and community-based setting.

Requirements

  • Bachelor's degree or Master's degree in psychology, social work, or in a related human services field.
  • A minimum of three years' experience working in adult mental health, addiction, and/or the jail.
  • Must successfully pass all required background checks.
  • Must have a valid NYS driver's license.
  • Must have reliable transportation.
  • Must have excellent verbal and written communication skills.

Responsibilities

  • Provides Jail based in-reach services.
  • Receives referral from Jail Transitions Coordinator or authorized referral source and initiates contact with identified client pre-release.
  • Completes ongoing risk/needs assessments including the COMPAS and Denver Acuity Scale.
  • Develops, in collaboration with the client, a transitions plan driven by the assessment and updates the plan based on the changing needs of clients.
  • Monitors the transitions plan to assure successful representation of the identified needs and adherence to goals of the plan.
  • Coordinates and facilitates ongoing meetings regarding the appropriateness of the care plan that include the client, natural supports, and any/all providers assisting in the delivery of the plan.
  • Assures that necessary supports are in place to assist the client carrying out the transitions plan.
  • Ensures that all of the initial linkages are established and maintained and advocates for additional services and linkages as appropriate.
  • Collaborates with all services providers establishing both a team communication plan and a crisis plan for the client.
  • Participates on the Multi-Disciplinary Reentry Case Management and Coordination Team.
  • Provides rehabilitative services identified on the plan of care which may include social and interpersonal skills, daily living skills, community integration.
  • Provides post release support, transportation, and assistance to appointments in the community.
  • Services are provided in a variety of community locations where the client lives, works, attends school, engages in services and/or socializes.
  • Provide services in a trauma informed, culturally and linguistically competent manner.
  • Complete all documentation in a timely manner.
  • Submits time sheet via payroll system in accordance with Agency regulations.
