About The Position
This is a full-time, community-based role for an Adult FACT Case Manager in Longview, WA. The position focuses on supporting adults in the community to build stability, independence, and a better quality of life. It involves hands-on work with real-world impact, where no two days are the same. The role emphasizes long-term stability and a team-based approach, offering the opportunity to build strong, meaningful client relationships.
Requirements
- High school diploma or equivalent
- 2+ years of experience working with individuals with mental health needs
- Ability to obtain WA Agency Affiliated Counselor within 7 days
- Valid driver’s license & reliable transportation
- Comfortable working in community settings
Nice To Haves
- Bachelor’s degree in Social Services or related field
- Experience in behavioral health or criminal justice settings
Responsibilities
- Work directly with clients in the community (homes, appointments, daily living settings)
- Support individuals with housing, employment, and life skills
- Help clients access benefits, healthcare, and community resources
- Teach daily living skills (budgeting, transportation, routines)
- Partner with a multidisciplinary FACT team
- Track progress and complete documentation
