About The Position

Teaches independent living skills to consumers and assists with personal care as needed. This role involves planning and conducting daily classes/activities, instructing clients on their Individual Program Plan (IPP) objectives, and adhering to program guidelines including Medicaid and DDS standards. The position also requires providing habilitation training, completing required in-service and out-of-area training, and potentially attending meetings after hours. Responsibilities include following the Policy and Procedures Manual, transporting clients, attending Annual Reviews, maintaining daily Medicaid notes and DOL information, and keeping classrooms and training areas clean. Documentation on Data Sheets, Progress Notes, AD Goal Sheets, and Young Homes daily log is required, as are Behavior, Seizure, and Incident Reports. The role involves supervising contract work, organizing supplies, providing Active Treatment Requirements for Young Homes clients, and complying with proper lifting techniques and hover/sling procedures. Maintaining a professional attitude and providing personal care needs are essential. The position also serves as a Driver for participant day trips, requiring periodic driving tests and potentially the use of a personal vehicle with adequate liability coverage. This is a safety-sensitive position that requires maintaining confidentiality in compliance with Friendship Community Care Non-Disclosure Policy and HIPAA Policies. The organization reserves the right to revise or change job duties as the need arises.

Requirements

  • High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
  • Ability to read and comprehend simple instructions, short correspondence, and memos.
  • Ability to write simple correspondence.
  • Ability to effectively present information in one-on-one and small group situations to customers, clients.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percentage and to draw and interpret bar graphs.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Employee is regularly required to use hands to finger, handle, or feel objects, tools, or controls and talk or hear.
  • Employee frequently is required to stand, walk, and reach with hands and arms.
  • Employee is frequently required to sit; climb or balance; stoop, kneel, crouch, or crawl; and taste or smell.
  • Employee must occasionally lift and/or move up to 50 pounds.
  • Specific vision abilities required by this job include close vision, color vision, depth perception, and the ability to adjust focus.
  • Required to undergo periodic driving test and demonstrate proficiency in the operation of a motor vehicle, including passenger vans.
  • Employees may also be required to drive his/her own personal vehicle at times and maintain adequate liability coverage on any personal vehicle used to transport participants.
  • Must follow the Code of Conduct and Compliance Investigation Procedures as read and discussed in the new employee training.

Responsibilities

  • Maintains needed communications with Immediate Supervisor.
  • Maintains written curriculum on weekly basis with advisement of Adult Development (AD) Manager.
  • Plans and conduct daily classes/activities with the consumers.
  • Instructs clients daily on their Individual Program Plan (IPP) objectives, using positive reinforcement and enthusiasm to complete goals.
  • Adheres to all program guidelines, including Medicaid, DDS, etc.
  • Provides habilitation training in life areas as required by DDS standards.
  • Completes in-service training required by DDS and/or Administration and out-of-area training, as required.
  • Attends in-house training as required by supervisor. Meetings may be after hours.
  • Assures Policy and Procedures Manual is followed.
  • Transports clients during training hours, as needed.
  • Attends Annual Review on all Adult Education clients, upon request.
  • Maintains daily Medicaid notes on each client, as applicable.
  • Maintains daily DOL information for individuals who work.
  • Maintains/cleans classrooms, training areas, and kitchen as directed by AD Coordinator.
  • Maintains daily documentation on Data Sheets, Progress Notes, AD Goal Sheets, Young Homes daily log, etc., as required.
  • Completes Behavior, Seizure, and Incident Reports, as needed.
  • Supervises Contract work/Quality of products made by clients, when scheduled.
  • Assists in keeping all class supplies/materials organized and replenished.
  • Provides Active Treatment Requirements for all Young Homes clients.
  • Complies with proper lifting techniques and hover/sling procedures.
  • Always maintains professional attitude/manner when dealing with clients, families, co-workers, etc.
  • Responsible for personal care needs of clients, as needed.
  • Serves as Driver for participant day trips to various state locations.
  • Maintains confidentiality of all information received regarding Friendship Community Care, Inc. and the clients.
  • Consumers’ progress notes be maintained and completed daily or as needed on the prescribed forms.
  • Consumers’ records must be maintained weekly, monthly, quarterly, and yearly.
  • All information must be filed in consumers’ records as needed and updated as prescribed by funding sources.
