About The Position

The Therapy Aide or Program Aide is a Health Care Aide (HCA) that assists Clients with activities of daily living to address personal, social, physical, and recreational needs. Performs all duties in a safe, caring manner according to the Clients’ wishes and Park Place policies and procedures. The aim of care is to promote independence and optimal functioning of Client’s mind, body, and spirit. Treats Clients with dignity and demonstrates an attitude of caring. The HCA ensures a commitment to Client safety and to our “culture of safety” as per Park Place Strategic Directions.

Requirements

  • Graduation from a recognized Health Care Aide program
  • Completion of Medication Assist through HCA Certificate or equivalent.
  • Ability to work in a structured program, able to adapt daily to changing needs or wishes of clients and have a genuine interest in caring for seniors.
  • Must be enthusiastic, outgoing, eager, and willing to learn and accept new responsibilities.
  • Communicate effectively in English, both verbally and in writing
  • Demonstrate patience, kindness, and compassion
  • Highly organized and resourceful
  • Operates related equipment
  • Works in a team setting
  • Collaborates and communicates with others in a respectful manner
  • Physical ability to carry out the duties of the position

Responsibilities

  • Provides care and support to Clients following their individualized care plans.
  • Establishes and maintains therapeutic relationships with the Clients and their family using respectful communication ensuring Client choice and autonomy in decision-making and care planning.
  • Ensures that Clients rights are maintained and respected, which includes confidentiality.
  • Provides holistic personal care to Clients, ensuring that the Clients activities of daily living needs are met as per Park Place policy and procedure.
  • Encourages and assists Clients to participate in social, physical, spiritual, and recreational goal directed activities offered in the program and implements safe and appropriate modifications to program activities to meet the individual needs of clients when needed.
  • Encourages and adheres to the Park Place philosophy of care, recognizing that each individual is capable of, to varying degrees; making decisions about his/her life and is a partner in the decision making.
  • Provides personal care to Clients, as needed, including but not limited to assisting the Client with mobility and transfer needs, assisting with toilet needs, assisting with ensuring nutritional and hydration needs of the Client are met while attending the program.
  • Following individualized Client Care Plan for safe transfers and ensuring Clients maintain good body alignment and comfort.
  • Assists Clients with ambulation as needed within the Home.
  • Contributes to the development and ongoing changes to the Clients’ individualized plan of Care. Advocates for the interest of the Client.
  • May be assigned Medication Assistance activities including documentation under the direction of a regulated health care provider (e.g., nurse) and in accordance with Park Place policies, procedures, and provincial standards.
  • Participate in required training and education programs, including Medication Assistance Program (MAP) certification and annual competency evaluation, prior to performing these duties.
  • Observes and monitors Client status, symptoms and behavioral changes and reports observations and all relevant information about care to the Program Manager.
  • Documents all relevant information on the Clients chart prior to the end of shift.
  • Assists Clients with menu selection, preparation and serving of snacks and meals, collection of trays and cleanup of dining area after meals. Monitor intake of nutrition and hydration and reporting of changes to the Program Manager.
  • Performs related clerical duties such as making and receiving phone calls.
  • Assists in the orientation and sharing information with new employees, students, and other members of the interdisciplinary team.
  • Actively Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings, and committees.
  • Maintains a safe environment for Clients, and employees by adhering to established safety policies, standards and procedures including emergency procedures, hand hygiene and all other infection control guidelines.
  • Ensures that clients are involved in identified recreational activities to meet the goals of the client’s care plan.
  • Leads, instructs, and motivates clients in individual and group leisure pursuits.
  • Apply motivational techniques to encourage participation.
  • Responsible for seasonal and special event decorations and cleanup.
  • Ensures proper care and labeling of Client’s personal effects, including clothing, purses using tags provided and returning tags when client leave program for the day
  • Take and record Client vital signs and weights as scheduled
  • Ensures care equipment and supplies are used and stored in safe and efficient manners.
  • Maintains a clean, welcoming environment for Clients and employees.
  • May complete duties as described in the Rehabilitation Aide job description
  • Performs other related duties as assigned.
  • Responsible to read, understand and comply with Park Place and provincially mandated Occupational Health and Safety policies and safe work practices.
  • Involved in all aspects of the Health and Safety Program including: Uses safe work procedures, Promotes health and safety policy and awareness, Makes safety suggestions, Reads OHS minutes posted.
  • Responsible to take every reasonable precaution to protect the safety of Clients, self, other employees, and the public.
  • Reports any near miss, injury, and accident or equipment damage to supervisor immediately and completes required reports.
  • Corrects and/or reports unsafe conditions.
  • Sets a good example.
