About The Position

Ebenezer is looking for an Adult Day Program Assistant to join their team. This position encourages socialization and assists in planning and implementation of programming needs of individual participants and groups, and assists with personal care duties. The role is responsible for ongoing charting and case management for their Skilled Nursing Facility in Bloomington, MN. The schedule includes 80 hours per two weeks, day shift from 8:00 am to 4:30 pm, with no weekends. Martin Luther Care Center, part of the Martin Luther Senior Living Campus, is located in East Bloomington, near woodlands/wetlands and along the Minnesota River Bluffs. Managed by Ebenezer, it offers 137 units across Skilled Nursing, Memory Care, and Transitional Care. The center has an outstanding team of tenured staff and was named a Top Nursing Home and Transitional Care Center by US News and World Reports for the past two years.

Requirements

  • CPR certification
  • Basic First Aide Training

Responsibilities

  • Assist the Director in the efficient operation of the program, including conducting tours, meeting with families, communicating with Case Managers, overseeing charting, assisting with care plans and assessments, assisting with office duties, and functioning as Lead staff in the absence of the Director.
  • Responsible for individual case management, including setting up participant charts, ensuring forms are completed and updated, maintaining monthly charting, quarterly care plans, and health updates, and documenting monthly health education, fire drills, blood pressures, and weights.
  • Provide direct care for Adult Day Program participants, including preparation, implementation, and cleanup of daily programs (crafts, woodworking, games, music, etc.), providing snacks and noon meals (including setup, food preparation, feeding, and cleanup), assisting staff in developing monthly calendars, and daily menu planning and ordering supplies.
  • Perform routine personal care and grooming, including baths, and assist identified participants with all toileting needs.
  • Promote and encourage individual care, provide individual and family counseling as needed, communicate participant needs and plans of care to staff, observe, assess, and intervene with participant health concerns, and report problems, progress, and observations to the Director.
