This position acts as an advocate for needed community services when the consumer is in need of concrete services. Core elements of this position include: (1) service plan development; (2) linking and coordination services; (3) advocacy and education provided to consumer to assess needed services and obtain concrete needs; (4) re-assessment and follow-up; (5) monitoring, linking, and coordinating of all services provided to the consumer. The person in this position works with a great deal of professional independence under the general guidance and supervision of the Adult Outreach Services Manager.
