About The Position

Case Managers under the supervision of their manager, must have the drive and desire to provide advocacy, arrangement and coordination of services, and linkage of resources based on the clinical needs of the client. The case manager has to be willing to meet the patient at their level of care and work towards a level of care that satisfies the patient's social recovery.

Requirements

  • High School Diploma or GED with 3+ years of relevant experience in case management, peer support, or behavioral health
  • Valid Driver's License and reliable transportation

Nice To Haves

  • Associate's or Bachelor's degree in Social Work, Psychology, Human Services, or a related field
  • Master's degree in a related field
  • Qualified Behavioral Health Specialist (QBHS) or QHMS certification
  • Chemical Dependency Counselor Assistant (CDCA) certification

Responsibilities

  • Provide community-based case management to clients with substance use and cooccurring mental health disorders.
  • Assess clients' clinical and social needs, ensuring they receive appropriate care, services, and referrals.
  • Develop, implement, and monitor individualized service plans (ISPs) in collaboration with the treatment team.
  • Link clients to housing, employment, education, food assistance, medical care, and other essential services.
  • Collaborate with psychiatrists, therapists, group facilitators, medical providers, and other professionals to ensure a holistic approach to client care.
  • Assist clients in accessing medication-assisted treatment (MAT) and other evidencebased interventions.
  • Advocate for clients' rights within social services, legal systems, and medical institutions as needed.
  • Conduct Substance Use Disorder (SUD), Therapeutic Behavioral Services (TBS), Psychosocial Rehabilitation (PSR), and Community Psychiatric Supportive Treatment (CPST) sessions.
  • Facilitate group activities that enhance self-sufficiency, resilience, and social skills.
  • Promote healthy coping strategies, relapse prevention techniques, and life skills development.
  • Address crisis situations and provide immediate intervention when needed.
  • Maintain a caseload of 20-30 clients, ensuring consistent support and engagement.
  • Attend weekly staff meetings and actively contribute to case reviews and team discussions.
  • Communicate professionally and effectively with all Vital Health staff and external service providers.
  • Adhere to all federal, state, and agency regulations to maintain compliance and service quality.
  • Uphold confidentiality standards by safeguarding client records and personal health information (PHI).
  • Maintain detailed, accurate, and timely case notes, ensuring documentation is completed within 24 hours of service.
  • Meet productivity expectations of 26 billable hours per week while maintaining service quality.
  • Monitor and document clients' progress, setbacks, and treatment adherence, adjusting service plans as needed.
  • Assist clients with transportation to appointments, recovery meetings, and essential services when necessary.
  • Engage in professional development and training to enhance case management skills.
  • Perform other duties as assigned to support the mission of the agency.
