About The Position

The Opportunity Alliance is seeking compassionate, organized, and dedicated Care Coordinators (Case Managers) to join their team-based case management model called Behavioral Health Home and make a meaningful impact in the lives of clients. This role involves working closely with individuals and families, connecting them to vital resources, advocating on their behalf, and supporting their journey toward stability and self-sufficiency. Care coordinators will work alongside a peer, nurse care manager, and medical/psychiatric consultants to provide holistic care coordination for adults with mental health and possibly medical needs. They will meet one-on-one with clients to assess and support their needs, with the support of an integrated care team. The organization emphasizes a supportive team culture, with 60% of the team having been with the company for 5 years or more. They offer a high level of supervision and support, including individual and group supervision, with no on-call, night, or weekend expectations. Clinical management supports work/life balance and manageable workloads. The ideal candidate will enjoy a direct helping role, supporting adults to meet their goals, problem-solving, forming relationships, and meeting with diverse groups of people. Organization and documentation skills are important.

Requirements

  • High school diploma or equivalent with one year of relevant adult behavioral health work experience as an MHRT-1 required.
  • Must have experience doing direct service with adults who struggle with mental health.
  • One year of experience required.
  • Must be able to successfully pass a criminal background, child protective service check & sex offender check.
  • Must not be on the state or federal suspension and disbarment list.
  • Must be able to exert moderate physical effort with occasional standing, walking, reaching, stooping, bending, kneeling, crouching, frequent typing and lift and carry material weighing up to 25 pounds.
  • Driving may be necessary to attend meetings and trainings and for the occasional need to meet with clients offsite and within the program service area.
  • A good driving record is required.

Nice To Haves

  • Associate’s Degree in the field of mental health, psychology, or a related field (behavioral health or human services) or a combination of education and experience that qualifies the candidate to receive an MHRT-C certification preferred.
  • MHRT-C certification or (level A or B with a strategy to receive full certification).
  • Prefer case management experience with adults who have serious and persistent mental illness and have a history of trauma, substance use, homelessness, poverty, chronic pain and serious health issues.
  • Two years of experience preferred.

Responsibilities
