About The Position

The Opportunity Alliance is seeking compassionate, organized, and dedicated Care Coordinators (Case Managers) to join their team-based case management model called Behavioral Health Home and make a meaningful impact in the lives of clients. This role involves working closely with individuals and families, connecting them to vital resources, advocating on their behalf, and supporting their journey toward stability and self-sufficiency. Care coordinators meet one on one with clients to assess and support their needs, while also having the support of an integrated care team, which includes a peer, nurse care manager, and medical/psychiatric consultants. The position is 40 hours per week, hourly, with a starting pay rate of $24.00/hour. The specific schedule will be discussed during the interview. The position is based in South Portland, ME, and serves Cumberland County.

Requirements

  • High school diploma or equivalent.
  • One year of relevant adult behavioral health work experience as an MHRT-1.
  • Must have experience doing direct service with adults who struggle with mental health.
  • Must be able to successfully pass a criminal background, child protective service check & sex offender check.
  • Must not be on the state or federal suspension and disbarment list.
  • Must be able to exert moderate physical effort with occasional standing, walking, reaching, stooping, bending, kneeling, crouching, frequent typing and lift and carry material weighing up to 25 pounds.
  • A good driving record is required.

Nice To Haves

  • Associate’s Degree in the field of mental health, psychology, or a related field (behavioral health or human services) or a combination of education and experience that qualifies the candidate to receive an MHRT-C certification.
  • MHRT-C certification or (level A or B with a strategy to receive full certification).
  • Case management experience with adults who have serious and persistent mental illness and have a history of trauma, substance use, homelessness, poverty, chronic pain and serious health issues.
  • One year of experience required, two preferred.

Responsibilities

  • Meet one on one with clients to assess and support their needs.
  • Connect clients to vital resources.
  • Advocate on behalf of clients.
  • Support clients' journey toward stability and self-sufficiency.
  • Work alongside a peer, nurse care manager, and medical/psychiatric consultants to provide holistic care coordination.
  • Perform documentation associated with the role.
  • Attend meetings and trainings.
  • Occasionally meet with clients offsite.
  • Potentially transport clients or drive to agency locations.
