The Adult Blended Service Coordinator serves as the primary point of accountability for adults receiving mental health services. This community-based role combines assessment, advocacy, outreach, service planning, and care coordination to help individuals achieve their recovery goals while addressing social determinants of health. This role is ideal for a self-starter who excels in a collaborative environment and demonstrates initiative, empathy, and strong communication skills. Adult Blended Service Coordinator serves adults (18+) throughout Allegheny County, including home, treatment locations, hospital, court, etc. Service Coordinator complete comprehensive assessments, service plan, and crisis plan. Service Coordinator links treatment services, resolves problems, and connects to community resources. Frequency of face-to-face contact with clients is determined by assessment – at least every 30 days. Clients qualify based on having a current mental health diagnosis, actively participating with service coordinator, and committing to utilizing treatment services. Typical service coordination caseload is 25-30 clients, depending on intensity of needs.
