About The Position

Milwaukee County is seeking authentic individuals to join their team as ADRC Professionals – Options Counselors within the Aging and Disability Resource Center. This role is crucial for connecting older adults and adults with disabilities with resources to live independently, safely, and healthily. The position involves assessing customer needs, determining functional eligibility for Long-Term Care programs, and providing comprehensive counseling. Success in this role requires extensive knowledge of resources and state regulations, as mandated by the ADRC contract with the State of WI.

Requirements

Current resident of the state of Wisconsin
Valid Wisconsin Driver's License and reliable insured personal vehicle
High School Diploma/GED equivalent with at least five (5) years of experience in Human Services, Case Management, or related field OR Associate degree in human services with at least two (2) years of experience in Human Services, Case Management, or related field OR Bachelor’s degree in human services with at least one (1) year of experience in Human Services, Case Management, or related field.
Intermediate knowledge of Microsoft Office Suite (Teams, Word, Excel)
Intermediate experience with CARES, ForwardHealth, FSIA, case management system
Must achieve Long-Term Care Functional Screen certification within one (1) month of employment.
Must achieve Options Counseling Standards certification within two (2) months of employment.
Knowledge of federal, state, and local laws and programs relating to social services of the target populations, community resources and services available to target populations.
Customer focus, excellent verbal communication skills, patience, ability to write succinctly, professionalism, critical thinking, and accountability.
Ability to work collaboratively with other units and organizations.
Proficient in data collection, analysis and in the use of general office equipment.

Nice To Haves

Ongoing certification is required by passing biennial examination for LTCFS.
Ongoing certification for Options Counseling is based on State regulations.

Responsibilities

Complete Options Counseling and achieve/maintain certification.
Determine functional eligibility for Wisconsin’s Long-Term Care programs.
Conduct Enrollment Counseling and Disenrollment Counseling.
Meet with customers, families, caregivers, and partner agencies to assess needs and goals.
Provide accurate information on resources and decision-support for navigating long-term care.
Follow-up with customers to ensure their needs are met.
Achieve Long-Term Care Functional Screen (LTCFS) certification within 1 month of employment and maintain it through ongoing training and exams.
Utilize motivational interviewing skills to complete functional assessments (Activities of Daily Living).
Score abilities, describe observations objectively in the Gateway/FSIA system, and review medical records related to functional deficits.
Complete 15 to 20 functional screens per month.
Possess extensive knowledge of WI’s Long-Term Care Medicaid programs (Family Care, Partnership, IRIS, and PACE), services, and eligibility.
Help customers understand programs without influencing their decision using DHS/state-approved publications.
Provide unbiased information on Managed Care Organizations and IRIS Consultant Agencies.
Assist IRIS customers in choosing a Fiscal Employer Agent.
Explain the financial and personal impact of enrollment or disenrollment from Long-Term Care Medicaid programs.
Accurately document all customer interactions in the case management system with case notes meeting Options Counseling standards.
Complete state-required encounter reports and maintain 100% time reports in 15-minute increments.
Maintain confidentiality and update customer records as needed.
Research public/private providers and programs to provide accurate information on eligibility, cost, services, and application methods.
Effectively explain complicated information to individuals with varying abilities.
Accurately respond to questions about financial eligibility, application process, cost share, patient liability, and income/asset impact.
Possess comprehensive knowledge of various Medicaid programs and waivers.
Participate in internal and external work groups, process improvement projects, county initiatives, and outreach events.
Contribute to best practice discussions.
Perform all other duties as assigned.
