This position is located within the Child and Family Care Coordination (CFCC) unit and works with youth with more intensive behavioral health treatment needs and their families. This position focuses on engaging families by assisting them in developing a plan of care for their child(ren); identifying, referring, and linking to appropriate behavioral health treatment; leveraging natural and community supports; coordinating services, monitoring progress, and providing updates to other systems including Court when applicable. The work hours may occasionally extend beyond the traditional 8:00am - 4:30pm workday.
