About The Position

Admissions Nurse (Start of Care SOC Visit RN) - Home Care: Enjoy independence, autonomy and flexibility in the field to care for patients one on one in their home environment. The service area where patients live is in the 11 County Twin Cities Metro area. Requirements

• Licensed to practice as an RN in the state of Minnesota. License is active and in good standing.
• Able to travel within geographic area serviced by the office.
• Valid driver's license, reliable transportation, and proof of auto insurance.
• Two years' experience as an RN.
• CPR Certification

Nice To Haves

• One (1) year of home care or hospice experience preferred.

Responsibilities

• Completes admissions or visits for the Home Care team.
• Direct patient care under the supervision of the Director of Nursing or Nursing Supervisor.
• Creating and/or following the plan of care.
• Provide and determine treatments for the patients.
