About The Position

Avalon Care Center in Bellingham is seeking an Admissions Nurse to join our team! This is a part-time position working Saturday - Sunday from 11:30 am to 7:00 pm. If you are advanced in problem solving, have great communication skills, and are highly organized then we want you to join our team! Requirements

Must have an RN in good standing
1-3 years of clinical nursing experience
At least two years in long term care
Two years of successful experience in administration, consultation or involvement in the survey process
Advanced in problem solving
Great communication skills
Highly organized

Responsibilities

Complete admission assessments for incoming residents, including physical and clinical evaluations.
Coordinate admissions to ensure a smooth transition into the facility.
Review hospital records, physician orders, medications, and treatment plans prior to admission.
Collaborate with physicians and interdisciplinary team members to support continuity of care.
Ensure accurate and timely completion of admission documentation, authorizations, and medical records.
Educate residents and families regarding admission procedures, care plans, and facility policies.
Assess residents for appropriate level of care and identify immediate clinical or safety concerns.
Coordinate room assignments, equipment, and resident care needs upon admission.
Maintain compliance with state, federal, facility, and HIPAA regulations.
Assist with resident transfers and discharge coordination as needed.
