About The Position

Arden Park Post Acute is seeking an experienced and compassionate RN Admissions Nurse to join our team on the PM shift. In this key role, you will coordinate new resident admissions, perform clinical assessments, oversee care transitions, and help ensure exceptional outcomes for our residents. The RN Admissions Nurse is responsible for coordinating and facilitating the admission process for new residents while ensuring accurate clinical assessments, documentation, regulatory compliance, and seamless transitions of care. This position works closely with physicians, hospital discharge planners, therapy teams, nursing staff, residents, and families to deliver a positive admission experience and maintain high-quality patient care.

Requirements

  • Active Registered Nurse (RN) license in California
  • Graduate of an accredited nursing program
  • Minimum 2 years of RN experience
  • Strong clinical assessment and care coordination skills
  • Excellent communication, organization, and documentation abilities

Nice To Haves

  • Experience in Skilled Nursing Facilities (SNF), Post-Acute Care, or Long-Term Care
  • Admissions nursing experience strongly preferred
  • Experience with PointClickCare (PCC) or other Electronic Health Record (EHR) systems
  • BSN preferred

Responsibilities

  • Coordinate and manage all aspects of resident admissions
  • Complete comprehensive nursing assessments upon admission
  • Review hospital records, physician orders, medications, and treatment plans
  • Ensure accurate and timely admission documentation
  • Verify insurance, authorization, and required clinical information
  • Facilitate smooth transitions from hospitals and other healthcare settings
  • Collaborate with physicians, nursing staff, therapy, pharmacy, and interdisciplinary teams
  • Communicate admission details and care needs to appropriate departments
  • Ensure timely implementation of physician orders and care plans
  • Coordinate laboratory services, diagnostic testing, and follow-up care
  • Act as a liaison between residents, families, and the care team
  • Assess resident conditions and identify changes in status
  • Administer medications, treatments, and IV therapy as needed
  • Respond appropriately to medical emergencies
  • Support residents in achieving the highest level of independence and well-being
  • Provide clinical guidance and support to nursing staff during the shift
  • Maintain accurate and complete admission records
  • Ensure compliance with federal, state, and facility regulations
  • Monitor documentation accuracy and regulatory requirements
  • Participate in quality improvement initiatives and audits
  • Maintain confidentiality and uphold HIPAA standards
  • Educate residents and families regarding care plans and facility services
  • Address questions and concerns throughout the admission process
  • Promote a welcoming and compassionate admission experience
  • Maintain professional and effective communication with residents and families
