About The Position

The RN Case Manager coordinates patient care from admission through discharge by collaborating with providers, healthcare teams, patients, families, and community partners to ensure safe, efficient, and patient-centered transitions. This role evaluates patient needs, develops discharge plans, and supports quality outcomes through effective care coordination and resource management.

Requirements

Current Colorado Registered Nurse DORA license required
Current American Heart Association BLS certification
Strong knowledge of professional nursing practices, patient care standards, safety principles, and healthcare regulations.
Skilled in communication, collaboration, and building effective relationships with patients, families, providers, and interdisciplinary teams.
Proficient in electronic health records, utilization review systems, Microsoft Office, and other computer applications used to support patient care.

Nice To Haves

BSN or equivalent preferred
Minimum of 5 years of nursing experience preferred
Case management or discharge planning experience highly desired

Responsibilities

Coordinate interdisciplinary care plans, admissions, discharge planning, and transitions of care to support positive patient outcomes.
Collaborate with providers, nursing staff, therapy, social services, outside agencies, and community resources to address patient needs and barriers to care.
Assess patient needs, including social determinants of health, and connect patients and families with appropriate resources and support services.
Monitor length of stay, discharge barriers, readmission risks, and opportunities to improve quality, efficiency, and continuity of care.
Maintain accurate documentation, participate in care coordination rounds, and support compliance with hospital policies, regulatory requirements, and nursing standards.
Performs a variety of tasks and other duties as assigned
