About The Position

The Admissions Coordinator is responsible for managing the facility admissions process, referral management and admission sign in as well as maintaining targeted occupancy levels of the facility. Meet census goals in conjunction with facility leadership and admissions liaison nurse. Requirements

  • Bachelor’s degree or NYS Nursing License required.
  • Strong computer skills required.
  • At least two years’ experience in a health care setting, preferably dealing with the elderly, with a working knowledge of Medicare, Medicaid and the long term health care system.
  • The individual must possess good moral character, patience, and compassion for the elderly, and be dependable, impeccable in grooming, tactful and able to retain information in the strictest confidence.
  • Must be a skilled communicator, director, motivator, detailed oriented and have customer service skills.
  • Possess a valid NYS driver’s license and have access to an automobile for work use.

Responsibilities

  • Responsible for managing and directing the admission process of a 120 bed nursing and rehabilitation facility.
  • Assists, receives and assesses inquiry calls from prospective patients/families and other referral sources.
  • Ability to observe and assess patient’s conditions and develop appropriate plan of care.
  • Conducts tours of the facility.
  • Builds and maintains relationships with multiple hospitals, assisted living and senior housing facilities.
  • Confirms insurance benefits and eligibility with multiple insurance companies.
  • Verifies dates of stay and financial data with other skilled nursing facilities in order to ensure information is accurate.
  • Obtains prior authorizations for all subacute admissions.
  • Checks all residents against the National Sex Offender list prior to admission.
  • Is the primary liaison with the hospital(s) and/or insurance companies.
  • Interview patients and provide education regarding requiring subacute rehabilitation, skilled nursing or hospice services on a daily basis.
  • Market the facility to potential referral sources.
  • Proficient in electronic medical records including Sorian (Catholic Health System), InfoClique (Kaleida Health), HealtheLink, and the facilities electronic health record.
  • Informs patients and/or responsible party with their financial responsibility prior to admission.
  • Assembles and distributes all admission paperwork to necessary departments.
  • Maintain records of all referrals and denials as per Department of Health guidelines.
  • Manages resident flow within facility including admissions, discharges and room changes.
  • Works with housekeeping and maintenance to ensure rooms are ready for new admissions.
  • Follows in-house residents who are admitted to the hospital and updates the facility on the resident’s status.
  • Attends daily meetings to inform all departments on new admissions, room changes or admissions with special needs.
  • Responsible for making sure the correct equipment is in house before a new admission arrives.
  • Verifies that all transportation and appointments for Dialysis are in place prior to admission of the resident.
