About The Position
Momentum at South Bay for Rehabilitation and Nursing is looking for a Full-time Admissions Coordinator. The ideal candidate will have good oral and written communication skills, interview and assessment skills, knowledge of entitlements and community resources, and knowledge of health care and gerontology issues. This role is responsible for maintaining the functions of the Admissions Department.

Responsibilities
- Conduct tours and provide information about Momentum at South Bay to applicants, hospital discharge planners and community agencies.
- Admit residents in accordance with established policies and procedures.
- Upon admission, responsible to complete, review and explain admission documents to residents and/or sponsor obtaining signatures as necessary.
- Complete pre-admissions in Sigma EMR.
- Other related duties as assigned by Admissions Director
