About The Position

Acts is currently seeking qualified candidates for our Admissions Coordinator/Social Worker position. In this role, you will be responsible for providing clinically related social services to support the well-being of our residents and their families. Participates in the admission process in collaboration with the Administrator and Director of Nursing. Assesses residents’ psychosocial needs and delivers individualized support services to help enhance their quality of life.

Requirements

• Bachelor's degree in Social Services.
• Minimum of two years' experience in a long-term care setting.
• Must be either a Licensed Certified Social Worker (LCSW) or Licensed Certified Social Worker-Clinical (LCSW-C), if employed in the state of Maryland.
• Compliance with Florida's Care Provider Background Screening process.

Nice To Haves

• Master’s degree is preferred.
