About The Position

Big Bend Hospice is seeking an experienced Registered Nurse and people leader to join us as our Admissions & Intake Team Manager. This position provides clinical and operational leadership for the centralized referral, intake, and admissions process supporting Hospice, Home Health, and Outpatient Palliative Care services. Requirements

  • Current, unrestricted Florida Registered Nurse (RN) license
  • Demonstrated clinical nursing experience
  • Leadership, supervisory, or team-lead experience
  • Strong understanding of clinical operations and patient-care coordination
  • Ability to lead, coach, and hold team members accountable in a fast-paced healthcare environment
  • Strong computer skills, including Microsoft Office and electronic documentation systems
  • Current BLS certification
  • Valid Florida driver's license

Nice To Haves

  • Two or more years of supervisory or healthcare leadership experience
  • Hospice, home health, palliative care, admissions, intake, referral, or care-coordination experience
  • Knowledge of Medicare, Medicaid, insurance benefits, and hospice eligibility requirements
  • Experience with healthcare quality improvement, documentation auditing, or performance management

Responsibilities

  • Provide day-to-day leadership, direction, coaching, and support to Admissions and Intake team members.
  • Train, mentor, and develop staff to strengthen clinical knowledge, performance, engagement, and professional growth.
  • Conduct regular one-on-one meetings, performance evaluations, competency reviews, and coaching conversations.
  • Manage scheduling, attendance, timecards, time-off requests, staffing coverage, and team resources.
  • Participate in interviewing, selecting, onboarding, and training new team members.
  • Promote accountability while creating a positive, collaborative, and supportive team culture.
  • Conduct clinical supervisory visits and identify opportunities for continued development.
  • Lead regular Admissions team meetings and communicate organizational and departmental changes.
  • Oversee daily centralized referral and intake operations for Hospice, Home Health, and Outpatient Palliative Care.
  • Ensure incoming referrals are processed accurately, efficiently, and within established timelines.
  • Monitor pending referrals and ensure appropriate follow-up and documentation.
  • Coordinate resources to support timely patient admissions.
  • Work collaboratively with the Admissions Supervisor, clinical teams, physicians, and on-call staff to prioritize admissions based on patient needs and acuity.
  • Resolve escalated referral, payer-verification, and intake concerns.
  • Help patients and families understand services and navigate the admissions process with compassion and professionalism.
  • Serve as a clinical resource and subject matter expert for the Admissions and Intake team.
  • Monitor admission activity, referral trends, conversion performance, and departmental outcomes.
  • Analyze referral data and identify opportunities to improve processes and team performance.
  • Perform documentation audits to support accuracy, compliance, and revenue integrity.
  • Participate in Quality Assessment and Performance Improvement (QAPI) initiatives.
  • Collaborate across departments to improve referral workflows and patient access to care.
  • Support the Certification of Need process and effective allocation of admission resources.
  • Identify operational barriers and implement solutions that improve efficiency and patient experience.
  • Ensure Admissions documentation is timely, accurate, and complete.
  • Evaluate patient appropriateness for hospice services in collaboration with physicians and clinical leadership.
  • Maintain knowledge of Medicare, Medicaid, insurance benefits, and applicable hospice requirements.
  • Educate and mentor team members regarding symptom management, admissions processes, community resources, and financial resources.
  • Provide direct patient care and admission support as needed to maintain clinical competency and ensure timely service.
  • Coordinate with clinical staff to minimize delays in care and support a seamless transition into services.
  • Advocate for patients and families throughout the admissions process.
  • Build effective relationships with physicians, referral partners, community organizations, and internal departments.
  • Collaborate with the Medical Director and other clinical leaders to advocate for patients and families.
  • Partner with Training and Development to strengthen team education and clinical competency.
  • Represent Big Bend Hospice professionally at community, educational, marketing, and customer-service activities.
  • Participate in the On-Call Manager rotation and field call rotation as required.
  • Provide field/admission support, including after-hours, weekends, and holidays, when operational needs require it.
