About The Position

Vanderbilt University Medical Center (VUMC) is seeking a dedicated Admission Visit Registered Nurse for Home Care. This role operates 32 hours per week, specifically on weekends. The nurse will collaborate in establishing the home health plan of care, coordinate and implement care delivery, and anticipate patient needs across the continuum. Responsibilities include analyzing patient information to determine nursing diagnoses, utilizing the nursing process to evaluate progress, and applying interventions for effective outcomes. The nurse will also employ strategies to promote health and a safe environment. This position is part of the VHCS Clinical - Nursing department and serves patients in Cheatham, Davidson, Robertson, Rutherford, Sumner, Williamson, and Wilson counties.   • LIC-Registered Nurse - Licensure-Others
  • Relevant Work Experience
  • 1 year experience
  • Graduate of an approved discipline specific program

Responsibilities

  • Collaborates in establishing the home health plan of care.
  • Coordinates and implements care delivery while anticipating patient care needs across the continuum.
  • Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.
  • Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes.
  • Employs strategies to promote health and a safe and healthy environment.
  • Conducts primary care patient interviews, assesses the safety of the home environment and performs a comprehensive physical assessment.
  • Recognizes problems, asks questions and contacts the patient's primary care provider.
