About The Position

The Admissions Nurse is responsible for the clinical and operational oversight of the nursing admission process, with primary emphasis on coordinating new admissions and supervising nursing staff involved in the resident’s transition into the facility. This position directs admission-related resident-care activities, coordinates staffing and assignments, monitors employee performance, ensures regulatory and policy compliance, and promotes consistent, resident-centered care. The position serves as the clinical lead for new admissions by reviewing referral information, preparing the unit, completing or coordinating admission assessments and documentation, verifying orders and medications, communicating with residents and families, and ensuring a safe transition into the facility. The Admissions Nurse also performs charge-nurse and floor-nurse functions when necessary to meet resident and operational needs. The Admissions Nurse is delegated the administrative and clinical authority, responsibility, and accountability necessary to carry out the duties of the position within the individual’s license, scope of practice, facility policy, and applicable federal, state, and local requirements. Responsibilities outside the LPN scope of practice must be completed or directed by an RN.

Requirements

  • Graduate of an accredited school of nursing.
  • Current, active, and unrestricted Florida license as a Registered Nurse or Licensed Practical Nurse.
  • Must maintain an eligible background-screening status and all other credentials required by law and facility policy.
  • At least one year of nursing experience in a long-term-care, skilled-nursing, rehabilitation, hospital, or similar healthcare setting preferred.
  • Supervisory, charge-nurse, unit-management, or admissions experience strongly preferred.
  • Strong leadership, supervision, delegation, coaching, conflict-resolution, organization, and follow-through skills.
  • Ability to prioritize admissions, staffing, clinical concerns, documentation, and unit operations in a fast-paced environment.
  • Sound clinical judgment and the ability to make independent decisions within scope of practice and escalate concerns appropriately.
  • Effective verbal and written communication and the ability to work tactfully with residents, families, employees, providers, regulators, and the public.
  • Knowledge of long-term-care nursing standards, the nursing process, medication management, care planning, documentation, infection prevention, resident rights, and applicable federal and state requirements.
  • Ability to use the facility’s electronic medical record and other required clinical and administrative systems.
  • Works throughout resident-care and nursing-service areas and is subject to frequent interruptions and changing priorities.
  • May be required to work beyond scheduled hours, weekends, holidays, or during emergency conditions and may be subject to call-back.
  • May be exposed to infectious diseases, blood and body fluids, hazardous chemicals, odors, air contaminants, hostile behavior, and other healthcare-related hazards. Required protective measures must be followed.
  • Must be able to move throughout the facility; stand, walk, bend, twist, reach, push, pull, and assist with resident movement or evacuation; and lift or move at least 35 pounds with or without reasonable accommodation or mechanical assistance.
  • Must be able to communicate effectively, cope with the mental and emotional demands of the position, and work with ill, disabled, elderly, emotionally upset, or occasionally hostile individuals.

Responsibilities

  • Direct and supervise the day-to-day work of licensed nurses, certified nursing assistants, nurse aide trainees, and other assigned nursing personnel.
  • Develop daily assignments, adjust staffing based on resident acuity and unit needs, and promptly address call-offs, tardiness, missed assignments, and coverage concerns.
  • Conduct regular rounds throughout the unit to verify that staff are completing assignments, responding to call lights, following care plans, and providing care consistent with nursing standards and resident preferences.
  • Provide clear shift expectations, receive and give nursing report, communicate changes in resident condition, and ensure effective handoff between shifts.
  • Coach, counsel, and guide employees; document performance concerns; participate in disciplinary action; and make recommendations regarding retention, transfer, promotion, or separation in accordance with facility policy.
  • Complete or assist with timely performance evaluations, competency reviews, and annual competency testing for assigned staff.
  • Orient, train, and mentor new nurses and nursing assistants; verify that employees understand their duties, unit procedures, documentation requirements, resident rights, infection-control practices, and emergency procedures.
  • Hold staff accountable for professional conduct, attendance, dress code, confidentiality, customer service, and compliance with facility policies and job descriptions.
  • Promote a calm, respectful, compassionate, and teamwork-focused unit environment. Address staff, resident, and family concerns promptly and escalate significant matters to nursing leadership.
  • Collaborate with the Director of Nursing, Assistant Director of Nursing, Staffing Coordinator, physicians, rehabilitation, dietary, social services, activities, MDS, pharmacy, and other departments to coordinate resident care.
  • Review available referral, hospital, clinical, medication, therapy, and transfer information before admission; identify missing, conflicting, or high-risk information and promptly communicate concerns to the appropriate leader or provider.
  • Confirm that the assigned room, equipment, supplies, medications, oxygen, special precautions, dietary needs, and staffing resources are ready before the resident arrives.
  • Notify nursing and interdisciplinary staff of the resident’s expected arrival time, room assignment, diagnosis, level of care, precautions, special equipment, and immediate care needs.
  • Greet and receive new residents and families, provide a professional introduction to the unit, explain the admission process, orient them to the room and facility routines, and address immediate questions or concerns.
  • Complete and document the admission nursing assessment within required timeframes and within the nurse’s scope of practice. LPN observations or findings requiring RN assessment must be reported promptly to the RN.
  • Obtain and document baseline vital signs, skin condition, pain status, functional status, cognition, mood/behavior, fall risk, nutritional concerns, elimination needs, infection risks, and other required admission information.
  • Perform or coordinate medication reconciliation; compare transfer records, medication lists, pharmacy information, and provider orders; and resolve discrepancies before medications are administered.
  • Review, transcribe, enter, and verify physician orders in the electronic medical record and communicate new orders to pharmacy, dietary, therapy, laboratory, and other departments as appropriate.
  • Notify the attending provider of the resident’s arrival and any immediate clinical concern. Communicate with the responsible party and obtain required consents, authorizations, emergency contacts, and advance-directive information according to facility process.
  • Initiate the interim or baseline plan of care, including safety precautions, treatments, monitoring, restorative needs, dietary orders, therapy referrals, and resident-specific interventions.
  • Complete or coordinate all required admission forms, records, census information, transfer documentation, nursing notes, and notifications. Ensure the admission record is accurate, complete, signed, dated, and ready for required follow-up.
  • Provide a complete admission handoff to the assigned nurse and CNA team, including diagnoses, risks, medications, treatments, preferences, behaviors, code status, appointments, and urgent follow-up needs.
  • Follow up after admission to confirm that orders were carried out, medications were received, treatments were initiated, care-plan interventions were communicated, and resident/family concerns were addressed.
  • Participate in admission review, quality monitoring, readmission reduction, and process improvement activities. Identify recurring admission barriers and recommend corrective action.
  • Assess, observe, monitor, and document resident status in accordance with professional standards and the nurse’s scope of practice. Report and respond promptly to significant changes in condition.
  • Provide direct nursing care and assist with medication administration, treatments, wound care, tube feedings, catheter care, specimen collection, vital signs, restorative services, and other licensed-nurse functions as assigned and permitted.
  • Prepare and administer medications and treatments safely; verify resident identity; maintain accurate controlled-substance records; investigate and report medication or narcotic discrepancies; and follow stop-order and medication-disposal procedures.
  • Receive and accurately document telephone or electronic provider orders. Ensure orders are transcribed, implemented, communicated, and followed up in a timely manner.
  • Notify the provider and responsible party of changes in condition, accidents, incidents, transfers, deaths, or other reportable events as required.
  • Complete incident/accident reports, transfer forms, progress notes, skilled documentation, and other required clinical records in an accurate, timely, and descriptive manner.
  • Coordinate emergency transfers, routine appointments, diagnostic services, discharges, transportation, and post-mortem procedures as required.
  • Serve as charge nurse or provide floor coverage when needed, while maintaining primary responsibility for safe unit supervision and the admission process.
  • Review resident care plans and clinical records to ensure that needs, goals, interventions, and changes in condition are current and accurately reflected.
  • Ensure nurses and CNAs understand and follow each resident’s care plan, assignment, precautions, preferences, and required monitoring.
  • Audit nursing documentation, medication and treatment records, admission records, controlled-substance records, and unit documentation for completeness, accuracy, timeliness, and compliance.
  • Participate in MDS, care-plan, discharge-planning, risk-management, Quality Assurance (QA), and Quality Assurance and Performance Improvement (QAPI) activities as requested.
  • Track identified concerns through completion, implement corrective actions, and report unresolved or repeated problems to the Director of Nursing or designee.
  • Participate in facility surveys and inspections and provide requested documentation or explanations within the employee’s area of responsibility.
  • Protect the privacy and confidentiality of resident, employee, and facility information.
  • Ensure residents are treated with dignity, kindness, respect, and privacy and that care reflects resident choices and the right to refuse treatment.
  • Respond professionally to residents, families, visitors, staff, and providers. Investigate complaints or grievances within assigned authority, document actions taken, and report concerns to nursing leadership.
  • Immediately report suspected abuse, neglect, exploitation, mistreatment, injuries of unknown origin, or misappropriation of resident property in accordance with law and facility policy.
  • Provide education to residents and families regarding medications, treatments, disease processes, safety needs, self-care, discharge planning, and facility routines as appropriate.
  • Enforce infection-prevention practices, standard and transmission-based precautions, hand hygiene, personal protective equipment use, and exposure reporting.
  • Maintain a clean, safe, sanitary, and organized unit, including nurses’ stations, medication rooms, treatment areas, resident rooms, and supply areas.
  • Ensure staff use lifts, gait belts, and other safety equipment correctly and that only trained and authorized employees operate nursing equipment.
  • Verify emergency equipment availability and function; report unsafe conditions, equipment failures, missing labels, supply shortages, or medication concerns immediately.
  • Monitor supply and medication inventory, anticipate needs, prevent waste, and submit requests according to facility procedures.
  • Participate in fire, disaster, evacuation, and emergency-preparedness activities and assist with resident evacuation when required.
  • Attend required meetings, in-services, annual education, and continuing-education programs; maintain all required licenses and certifications.
  • Assist with education and in-service programs based on observed needs, policy changes, survey findings, incidents, admission trends, and performance concerns.
  • Maintain current knowledge of nursing standards, pain management, medication administration, adult and geriatric care, infection prevention, resident rights, and long-term-care regulations.
  • Support the facility’s Corporate Compliance and Code of Conduct requirements and perform other related duties as assigned or directed.
