About The Position

Hospice of Midland is seeking an Admission Liaison to receive referrals and physician orders, coordinate and triage calls from patients and families, explain hospice services, and complete admission requirements. The role also participates in community outreach and public relations with physicians, discharge planners, and other referral sources. The Admission Liaison additionally functions as a staff nurse to meet patient-visit needs. This is a non-exempt position with daytime availability required Monday through Friday, within 6 AM–5 PM.

Requirements

• Be available during daytime hours Monday through Friday within 6 AM–5 PM.

Responsibilities

• Receive referrals and physician orders.
• Coordinate and triage calls from patients and families.
• Explain services to patients and families.
• Complete admission requirements.
• Participate in community outreach and public relations with physicians, discharge planners, and other referral sources.
• Function as a staff nurse for patient visit needs.
