Admission Coordinator
About The Position
Join our growing team as an admissions coordinator! This is a full-time position (40 hours/week) based at our Eau Claire – West location, serving multiple Dove Healthcare locations. Dove Healthcare consists of 11 skilled nursing and rehabilitation centers, six assisted living residences, and three independent living complexes across Wisconsin. Our core services include inpatient and outpatient rehabilitation, post-hospital care, ventilator care, skilled nursing, long-term and end-of-life care, as well as senior independent living, assisted living, and memory care. As an innovative healthcare provider and community partner, we provide compassionate care and service to meet the needs of those who depend on us. Requirements
- Professional health care experience required (social worker, nurse CMA, CNA, pharmacy tech, PTA, COTA, etc.)
- Ability to multitask, juggle multiple priorities and work in a fast-paced environment
- Proficiency in relevant software tools / applications such as Microsoft
- Detailed oriented with strong organizational and time management skills
- Strong problem-solving abilities
- Proficiency in written and verbal communications
Nice To Haves
- Experience with EPIC/Care Anywhere, Point Click Care (CRM) is helpful.
Responsibilities
- Coordinating admissions with hospital discharge planners and other referral sources
- Assessing, coordinating and ensuring a smooth admission transition
- Confirms medical and financial information needed to ensure a smooth admission transition
- Develops and maintains excellent networking skills and relationship building with referral sources
- Ensuring patient continuity of care
