About The Position

The Administrative Care Manager – IC, under the direction of the VP of Clinical Services, supervises all day-to-day operations of the Interdisciplinary teams and participant outcomes. As such, the Administrative Care Manager serves as the Administrator for the LHCSA along with coordination of the multi-disciplinary integrated care in the community, Day Health Center, Diagnostic & Treatment Center, and throughout the continuum of care. Coordinates with all managers to ensure all metrics logs are complete and provide support and assistance as required.

Requirements

  • Baccalaureate degree in Business Administration, Healthcare Administration, or related fields.
  • Minimum of 3 years of recent healthcare operations experience.
  • Minimum of one (1) year of experience working with elderly population in a Certified Home Health Agency (CHHA), Managed Long Term Care (MLTC) and Program of All Inclusive Care for the Elderly (PACE).
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Nice To Haves

  • A master’s degree is preferred
  • Minimum of two to three (2 – 3) years of Clinical Staff Supervisory experience preferred.
  • Bilingual preferred.

Responsibilities

  • Facilitate Morning Report and SDR Meetings when needed.
  • Update Whiteboard.
  • Run reports (missing Aide POC, Expired orders, etc) and email CHNs for follow up.
  • Monitor CHN Calendar and email CHN for follow ups/updates.
  • Monitor open notes and email CHN to follow up.
  • Assign monthly Nursing visits (dialysis, NH, palliative care).
  • Audit Incident Reports for timely step 2 completion.
  • Audit Charts for compliance.
  • Audit CICs send reminders notes to indicate CIC, completed and Care Planned on time.
  • Audit IDT attestations for completion.
  • Disenrollment review and follow-up calls.
  • Run and monitor weekly productivity, assist CSD with preparing Goals report.
  • SDR step 6s.
  • Liberty dental denials.
  • HPMS ER/Urgent visits.
  • Incontinence supplies review for monthly updates.
  • Serve as a recourse to the care delivery team.
