About The Position

Care Dimensions is seeking an Administrative Assistant for its Palliative Care & Dementia GUIDE programs. This role supports growing programs dedicated to helping individuals and families navigate serious illness and dementia with dignity, guidance, and expert care. The Administrative Assistant will be a vital member of the team, ensuring patients and families receive timely support while keeping daily operations running smoothly. This role also supports the Physician Services team through cross-training and collaboration, providing administrative coverage and support across multiple service lines including Palliative Care, Dementia GUIDE, Hospice Physician Services, and HomeMD Primary Care, ensuring continuity of operations and exceptional service throughout the organization.

Requirements

  • Minimum of one year of experience in a medical office, healthcare setting, or related environment.
  • Demonstrated ability to coordinate communications and activities within a multidisciplinary healthcare setting.
  • Strong organizational skills with the ability to manage multiple priorities, work independently, and meet deadlines.
  • Proficiency in Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint, as well as electronic medical records and other databases.
  • Excellent customer service skills and a commitment to providing exceptional support to patients, families, and colleagues.
  • Strong interpersonal skills and the ability to work collaboratively as part of a team.
  • Ability to exercise independent judgment and maintain strict confidentiality when handling sensitive medical information.

Responsibilities

  • Respond to inquiries regarding Palliative Care and Dementia GUIDE services with professionalism, efficiency, and compassion.
  • Serve as a welcoming and knowledgeable resource for patients, families, referral sources, and community partners.
  • Coordinate daily operational activities and schedules for physicians, advanced practice clinicians, and support staff.
  • Register patients in the electronic medical record, verify insurance coverage and eligibility, and schedule appointments.
  • Support providers through patient assignment, visit scheduling, appointment coordination, and follow-up communication with patients and families.
  • Collaborate with clinical and administrative colleagues to ensure timely responses to patient and service needs.
  • Participate in cross-training opportunities and provide coverage for administrative functions within Hospice Physician Services and HomeMD Primary Care as needed.
  • Prepare, edit, and maintain reports, correspondence, presentations, forms, policies, spreadsheets, and other operational documents.
  • Complete administrative responsibilities accurately, efficiently, and with exceptional attention to detail.
  • Maintain confidential patient information while exercising sound judgment and professionalism.
  • Foster strong working relationships across departments to promote teamwork, flexibility, and operational excellence.
