About The Position

A leading healthcare provider is seeking an Acute Rehabilitation Physical Therapist to evaluate and treat patients of all ages, develop and modify individualized plans of care, deliver high-quality physical therapy services, serve as a clinical resource for staff and students, and promote excellence in patient care through professional practice and collaboration.

Requirements

Bachelor's degree in Physical Therapy.
Current Illinois Physical Therapist (PT) License.
Basic Life Support (BLS) certification through the American Heart Association (AHA).
Knowledge of physical therapy evaluation techniques, treatment planning, rehabilitation principles, and patient care.

Responsibilities

Evaluate and treat patients of all ages by performing comprehensive physical therapy assessments and developing individualized treatment plans based on patient needs and clinical findings.
Modify treatment plans and interventions as appropriate to support patient progress, functional improvement, and optimal rehabilitation outcomes.
Deliver physical therapy services while ensuring high-quality, patient-centered care and adherence to professional practice standards.
Serve as a role model in the delivery of professional physical therapy services and provide clinical guidance and support to staff and students.
Collaborate with interdisciplinary healthcare teams to coordinate patient care and promote effective communication throughout the rehabilitation process.
