About The Position

The Acute Clinical Liaison is responsible for coordinating acute clinical services, ensuring optimal patient outcomes, and supporting healthcare staff in patient transitions to the home. The role combines clinical expertise with sales support, requiring communication skills to foster relationships and assist in driving business growth. The role typically involves frequent visits to hospitals and acute care facilities, interaction with diverse clinical teams, and occasional after-hours support. It requires adaptability, professionalism, and a strong commitment to patient care and sales excellence.

Requirements

  • Bachelor’s degree in nursing, healthcare, life sciences, or related field.
  • Minimum of 2–3 years of clinical experience in an acute care setting (such as ICU, ER, or hospital).
  • Strong clinical knowledge, excellent communication and presentation skills, ability to build and maintain relationships, and proficiency in problem-solving.
  • Basic level skill in Microsoft Office (including Word, Excel, PowerPoint, etc.).
  • Travel for business purposes in a specific territory geography.
  • Must be a graduate of an N.L.N. accredited school of nursing.
  • Must hold a current license as a Registered Nurse in the state in which you are currently practicing.
  • Must have or obtain a current CPR certificate within the first 30 days of employment.
  • Must have valid driver’s license in state of employment and willingness to travel within assigned territory.

Responsibilities

  • Primary individual in market for teaching demonstration of infusion to patient and/or caregiver, initiating patient and/or caregiver teaching in the hospital including flush procedures, pump usage/troubleshooting, IV therapy administration, etc.
  • Accurately prepare, administer and monitor pharmacological agents, if applicable.
  • Maintain thorough and proper documentation in the patient medical records of all teaching performed as well as phone contacts with the patient, physician, or other members of the health care team.
  • Obtain all proper discharge documentation, referral information and aid billing personnel in verification of payment prior to hospital discharge.
  • Complete home care record documentation, if applicable.
  • Work with Acute Clinical Sales Liaison (ACSL) and proper Admissions and Nursing partners to coordinate and facilitate referral prior to patient discharge.
  • Communicate and/or coordinate all services and equipment patients may require at home in addition to IV therapy (i.e., DME equipment) from hospital or MD office setting.
  • Validate physician’s or authorized prescriber’s orders (as applicable).
  • Document and complete referral management within the proper systems.
  • Accurately and completely assess the patient and their environment for appropriateness for homecare.
  • Accurately observe and assess psychosocial issues, adverse reactions and complications and coordinate proper interventions.
  • Provide patient and/or caregiver with information on the concept of home infusion therapy and the impact of such therapies.
  • Verbalize indications of therapy, pharmacological actions, interactions, side effects and monitoring parameters.
  • Communicate with ACSL on patients that become rehospitalized.
  • Support Acute Clinical Sales Liaison (ACSL) in market to identify and pursue new business opportunities within acute care facilities by collaborating with physicians, nurses, pharmacists, hospital case managers, discharge planners and social work staff.
  • Support pharmacy/company to meet quarterly and annual growth objectives.
  • Support relationships with physicians, nurses, pharmacists, hospital case managers, discharge planners and social work staff by aiding discharge planners in the referral processes and preparing patients requiring home infusion therapy, for discharge.
  • In times when patient education and teaching demonstrations are not demanded, participate in weekly patient care rounds and patient care conferences.
  • Work with ACSL to educate healthcare professionals on the clinical benefits, usage, and processes of acute care services.
  • Provide in-service training, demonstrations, and ongoing support to ensure proper implementation and patient safety.
  • Aid in communicating with customers and internal partners to ensure seamless patient transitions and referral processing.
  • Ensure that patient needs are met with a focus on quality and continuity of care.
  • Ensure all activities adhere to regulatory standards, company policies, and ethical guidelines.
  • Maintain clinical expertise and fulfill licensure requirements (as applicable)
  • Other duties as assigned.
