About The Position

The LTACH Clinical Liaison is responsible for transitioning patients from the
acute care environment to Mosaic LTACH, St. Joseph. The Clinical Liaison will
advocate for referred patients in the pre-admission process by collaborating
with discharge planners to assess patient needs. The CL will serve as a resource
for healthcare professionals through fostering relationships within medical
facilities. With clinical expertise and problem-solving skills, the CL will
coordinate care plans with physicians and families. Additionally, the CL will
assist in the facilitation of patient transfers and address barriers to
admission. The CL will develop and maintain new and current relationships with
various areas of Mosaic Life Care Hospital, Clinics and PAC providers. This
position reports to the LTACH President.
