About The Position

Kidney Specialists of San Antonio, part of the Panoramic Health network, is seeking a dedicated Acute Care Nurse Practitioner (AGACNP) to join our growing nephrology team. Our team provides comprehensive nephrology care across outpatient clinics, dialysis centers, and hospital settings. You will work alongside experienced nephrologists and advanced practice providers who are committed to mentorship, collaboration, and clinical excellence. The Acute Care Nurse Practitioner will work collaboratively with physicians and care teams to evaluate, diagnose, and manage patients with acute and chronic kidney disease. This role includes hospital rounding, dialysis management, and outpatient follow-up care.

Requirements

  • Master's or Doctorate degree from an accredited Nurse Practitioner program
  • Current Texas Nurse Practitioner license or eligibility for licensure
  • Acute Care Nurse Practitioner certification (AGACNP or ACNP) required
  • Current DEA license or ability to obtain
  • Current BLS certification

Nice To Haves

  • Nephrology, hospital medicine, critical care, internal medicine, or dialysis experience
  • New graduates with relevant clinical training and a strong interest in nephrology are encouraged to apply

Responsibilities

  • Conduct comprehensive patient assessments and develop treatment plans
  • Manage patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), hypertension, electrolyte disorders, and other nephrology-related conditions
  • Perform hospital rounds and coordinate care with physicians and interdisciplinary teams
  • Monitor dialysis patients and assist with treatment management
  • Order, interpret, and communicate diagnostic tests and laboratory results
  • Provide patient and family education regarding disease management and treatment plans
  • Document patient encounters accurately and timely within the electronic medical record
  • Participate in quality improvement and patient care initiatives
