Houston Kidney Specialists Center (HKSC) is a growing, physician-led nephrology practice dedicated to delivering exceptional kidney care throughout the greater Houston area. With multiple state-of-the-art clinic locations and a team of experienced nephrologists, HKSC provides comprehensive care for patients with chronic kidney disease, hypertension, dialysis needs, and related conditions. We are seeking an Acute Care Nurse Practitioner (AGACNP) to join our collaborative nephrology team. Requirements
- Master's or Doctoral degree in Nursing from an accredited program
- Current Acute Care Nurse Practitioner certification (AGACNP)
- Active Texas Nurse Practitioner license or eligibility to obtain
- Minimum of one year of Nurse Practitioner experience
- Strong clinical assessment, diagnostic, and care management skills
- Excellent communication and collaboration abilities
- Commitment to delivering high-quality, patient-centered care
Nice To Haves
- Nephrology or dialysis experience preferred, but not required
- Bilingual Spanish/English preferred
Responsibilities
- Evaluate, diagnose, and manage patients with acute and chronic kidney disease
- Collaborate closely with nephrologists and interdisciplinary care teams
- Develop and implement treatment plans to improve patient outcomes
- Provide patient education regarding kidney health, treatment options, and disease management
- Document patient encounters accurately and efficiently within the EMR
- Participate in care coordination across outpatient and inpatient settings
- Manage a small subset of dialysis patients as part of the practice's comprehensive care model
