About The Position

The Pharmacist provides quality pharmaceutical care, ensuring the safe, effective, and cost-efficient use of medications for all patients. This role involves developing, recommending, and monitoring evidence-based pharmacotherapy care plans, with a focus on patient outcomes and quality of life. Responsibilities include assessing patient medications for indication, effectiveness, safety, and adherence, as well as monitoring patient and laboratory parameters and documenting outcomes.

Requirements

  • Bachelor's Degree in Pharmacy (Required)
  • Licensed Pharmacist (Pennsylvania) within 90 days
  • Ability to coordinate the use of human, material, and informational resources.
  • Ability to review patient medication profiles.
  • Ability to calculate drug dosages.
  • Ability to prepare special medications involving compounding procedures and sterile products using aseptic techniques.
  • Ability to coordinate and supervise work-place activities and resources.
  • Ability to advise, assist, and supervise the work of pharmacy technicians, students, and pharmacy interns.
  • Ability to work with medical and nursing personnel to resolve problems.
  • Ability to coordinate activities with other pharmacy personnel.
  • Ability to assist with the review and revision of departmental policies and procedures.

Responsibilities

  • Assumes responsibility for the delivery of pharmaceutical care to the specific patient care area.
  • Coordinates the use of resources for timely, efficient, cost-effective, and appropriate care.
  • Receives and interprets physicians' medication orders, clarifying or correcting them with the physician.
  • Reviews patient medication profiles for potential issues like drug sensitivities, contraindications, duplications, and major drug interactions.
  • Reviews all medication orders for prescribing, formulary, and therapeutic issues.
  • Suggests appropriate formulary alternatives based on patient clinical parameters.
  • Consults and advises medical staff on drug therapy problems and monitors patient laboratory results.
  • Provides drug information to medical, nursing, and other healthcare professionals.
  • Provides patient discharge medication consultations.
  • Makes rounds with medical services.
  • Calculates drug dosages based on pharmacokinetic parameters.
  • Verifies provider-entered orders and ensures proper doses are dispensed.
  • Provides in-services to other healthcare providers when requested or necessary.
  • Prepares special medications, including compounding and sterile products using aseptic techniques.
  • Reports requirements for additional supplies and equipment.
  • Coordinates and supervises workplace activities and resources, promoting professional competence.
  • Advises, assists, and supervises pharmacy technicians, students, and interns, assuming final responsibility for their work's accuracy.
  • Works with medical and nursing personnel to resolve pharmacy service issues.
  • Coordinates activities with other pharmacy personnel.
  • Assists with the review and revision of departmental policies and procedures.
  • Accountable for satisfying all job-specific obligations and complying with organizational policies and procedures.
