About The Position

Clarity Care, a nonprofit organization, is dedicated to supporting individuals with varying abilities to lead independent, fulfilling lives within their communities. We are seeking compassionate individuals to join our team as Activity Support Providers. This role assists with the day-to-day operations of the Day Services Program, focusing on goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance. Requirements

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards.
• Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
