About The Position

Clarity Care, a nonprofit organization dedicated to supporting individuals with varying abilities, is seeking an Activities Support Provider PT to join their team. This role focuses on empowering individuals to lead independent, fulfilling lives within their communities. The organization is committed to providing compassionate, high-quality care with respect, integrity, and a deep commitment to the people they serve. The Activity Support Provider assists with the day-to-day operations of the Day Services Program, focusing on goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance. No experience or certifications are needed, and the organization is hiring immediately.

Requirements

• High school diploma/equivalent and/or related work/life experience
• At least 18 years of age
• Valid WI driver’s license, and ability to meet agency driving standards
• Proof of auto insurance
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence
