About The Position

Clarity Care is seeking compassionate individuals to join their team as Activity Support Providers. This nonprofit organization is dedicated to supporting individuals with varying abilities to lead independent, fulfilling lives. The Activity Support Provider will assist with the day-to-day operations of the Day Services Program, focusing on goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance. Requirements

• High school diploma/equivalent and/or related work/life experience
• At least 18 years of age
• Valid WI driver’s license, and ability to meet agency driving standards
• Proof of auto insurance
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence
