About The Position

Elderwood Village at St. Gregory Court is seeking an Activities Program Manager for our state-of-the-art Memory Care facility. This role involves leading a meaningful and engaging activities program designed to enrich the lives of residents in a Special Needs Assisted Living Residence. The position combines program leadership with team development, aiming to create opportunities for residents to connect, stay active, learn new skills, and thrive while supporting their individual needs and interests.

Requirements

  • Completion of the Modular Education Program for Activity Professionals (MEPAP); or a bachelor’s degree in recreation therapy; or is an occupational therapist, COTA, CRT, AT-BC (art therapist), MT-BC (music therapist) or has another clinical degree.
  • Certification in dementia and/or behavior management (e.g. CDT) required.
  • Demonstrated computer skills including Microsoft Office (Word, Excel) and web-based applications.
  • Ability to read, write, speak, and understand the English language at an intermediate or more advanced level.

Nice To Haves

  • Certification as a Dementia trainer in Crisis Prevent Management (within first year of hire).
  • Qualified Therapeutic Recreation Specialist or an Activities professional preferred.
  • Experience in state and federal requirements and documentation preferred.
  • Demonstrated supervisory experience preferred.

Responsibilities

  • Developing and directing an activity program for the leisure time of Special Needs Assisted Living Residence (SNARL) residents, designed for entertainment, relaxation, socialization, exercise, stimulation, skill development, and enhancement of individual, psychological, cultural, physical, social, and spiritual needs.
  • Training, supervising, and monitoring the work progress of site Activity Leaders, Memory Care Program Specialist, and overseeing HHAs and their assignments.
  • Planning, directing, and scheduling monthly activity programs, including a mixture of group, individual, and independent activities (cognitive, cultural, spiritual, diversional, physical, political, social, and intellectual interests) geared toward individual and group residential interests and capabilities.
  • Interviewing new residents, preparing initial resident assessment information, and ensuring resident service plans are appropriately updated.
  • Reviewing and/or attending Resident/Family Care Plan Review sessions, and the medical record chart of each resident on a quarterly/monthly/annual basis, ensuring required documentation is completed and in residents' files; writing and/or reviewing activities, ADL, and behavior plans of care, preparing and/or reviewing required documentation including initial assessments, progress notes, and attendance during activity programs in the Medical Record Chart of each resident.
  • Managing and developing activities within the Seasons model.
  • Providing dementia certification instruction to staff and training staff on programming.
  • Educating HHAs and Activity Leaders for programming, as well as providing dementia certification instruction for all staff within the facility.
  • Supervising Activity Leaders and Memory Care Program Specialist, ensuring staff follow policies and procedures, adhere to the dress code, and support Elderwood’s Mission, Vision, and iCARE Values.
  • Directing and monitoring resident programming and specialized memory care approaches for personal care duties carried out by the HHA, while also providing feedback to the DON.
  • Planning Volunteer Programs and recruiting, orienting, training, and directing volunteers, cultivating and maintaining good relationships with community organizations.
  • Coordinating religious programs for residents and facility-wide entertainment events.
  • Serving as editor of the newsletter and collaborating with the Director of Memory Care and Programming, and Communications Coordinator in the production process.
  • Acting as a point of contact and providing support services to volunteers who lead weekly services, facility-wide events, and/or provide entertainment for such events.
  • Facilitating communication with dietary staff regarding diet requirements of residents and supplies needed for specific activity events.
  • Keeping records and submitting money to the Business Office for all income-producing activities; recording and submitting information regarding transactions of residents through Resident Account funds.
  • Overseeing and updating department policies and procedures annually.
  • Arriving to work on time, regularly, and working as scheduled.
  • Recognizing and following the dress code of the facility, including wearing a name tag at all times.
  • Following policy and procedure regarding all electronic devices, computers, tablets, etc.
  • Abiding by Elderwood’s business code of conduct, compliance, and HIPAA policies.
  • Performing other duties as assigned by supervisor, management staff, or Administrator.
