About The Position

The Activities Nurse is responsible for developing, planning, coordinating, and implementing therapeutic and recreational activities that support the physical, emotional, cognitive, and social well-being of patients or residents. This role combines clinical knowledge with activity programming to enhance quality of life. This includes the completion and/or delegating the completion of the activities component of the comprehensive assessment (MDS), activities assessments, and contributing to and/or delegating the contribution to the comprehensive care plan and approaches that are individualized to match the skills, abilities, and interests/preference of each resident. Coordinating the activities program includes scheduling of activities, including individual and groups, implementing and/or delegating the implementation of the programs, monitoring the response and/or reviewing/evaluating the response to the programs to determine if the activities meet the assessed needs of the resident, and making revisions as necessary. This role will also include planning outside activities and getting the residents out in the community along with mentoring relationships with community entities. May require evenings, weekends, or holiday shifts depending on programming needs.

Requirements

  • Certificate of completion from college or vocational school Graduation from a National League of Nurse (NLN) Commission on Collegiate Nursing Education (CCNE) or equivalent accredited school of nursing
  • 6 months to 1 year relevant experience
  • California Registered Nurse (RN) Upon hire
  • Valid Driver's License in good standing Upon hire
  • Basic Life Support (BLS) for Healthcare Providers Upon hire
  • Demonstrates strong analytical skills; strong communication skills; excellent organizational skills with the ability to prioritize requests and assignments.
  • Ability to work independently and as part of a multidisciplinary team

Nice To Haves

  • Activities Director Certificate Within 6 months of hire into job
  • Experience in long-term care, geriatrics, or rehabilitation
  • Knowledge of activity programming, especially for elderly or cognitively impaired populations (e.g., dementia care)
  • Competence in Microsoft Excel, Word, and PowerPoint

Responsibilities

  • Assesses psycho-social needs of residents and reports to those findings to social services as needed.
  • Assesses activity needs of the Residents.
  • Identifies and plans individualized care of the resident that addresses both activities and social services.
  • Develops the individualized care plans of the residents.
  • Contributes to the individualized resident plan of care.
  • Understands and documents Minimum Data Set (MDS) accurately.
  • Documents timely and accurately resident assessments, issues and interventions.
  • Enters and retrieves data from computer system.
  • Organize special events, outings, and seasonal programs
  • Encourage social interaction and community involvement
  • Maintain activity supplies, equipment, and program records
  • Provides resource referrals.
  • Provides education information for resident and family.
  • Assists resident in obtaining special items.
  • Organizes and attends the Resident and Family Council Meetings.
  • Interacts with families to keep them informed on the resident’s psychosocial needs and wellbeing.
  • Review resident rights with residents quarterly.
  • Coordinates the scheduling of screenings and outside medical appointments.
  • Monitors and provides quarterly reports for the resident trust fund.
  • Meet, at a minimum, monthly, Social Worker to review monthly activity and/or resident issues.
  • Participates in resident care conferences.
  • Responsible for protecting self, facility, resident, and other members of the staff by following security policies and procedures.
  • Develops and implements an activity plan for the Residents, posting and distributing a monthly activity calendar.
  • Supervises and evaluates residents’ activity and plan.
  • Coordinates resources for an effective activity program.
  • Meets residents' needs for activities including physical exercise and mental exercise.
  • Identifies resident issues and utilizes chain of command to address.
  • Ensure activities comply with safety standards and infection control policies
  • Participates in departmental growth and development.
  • Participates as a member of the nursing staff within Extended Care Center and maintains all competencies as Staff Nurse Extended Care.
  • May fill in on floor in staffing if needed.
  • Demonstrates System Values in performance and behavior.
  • Complies with System policies and procedures.
  • Other duties as may be assigned.
