About The Position

The long term care (LTC) Activities Coordinator leads the planning and development of activity programming, and assures activities are lead in a safe and effective manner by staff. This role acts as a resource for employees, residents, and families regarding activities and related matters. It involves establishing relationships with outside community partners to ensure a positive experience. The coordinator creates and plans activities around established resident care plans, assesses resident needs, and develops programs to meet those needs. Responsibilities include completing activity assessments and Minimum Data Set (MDS) questions, and striving to improve residents' quality of life through appropriate, meaningful activities and interventions. The coordinator administers the monthly activity calendar, elicits resident input, and ensures activities are well-organized and upbeat, adhering to state requirements for music, craft, and spiritual activities. Participation in activities like fishing, nature walks, pet therapy, and music therapy is expected to support residents' optimal functioning, dignity, and identity. The role may involve assisting with simple nursing procedures and transporting residents. Oversight of programming effectiveness and providing feedback for enhancement are key. The position requires self-direction, a high level of accountability, and minimal supervision.

Requirements

  • High school graduate or equivalent required.
  • Must be eligible for certification as a therapeutic recreation specialist or activities professional by a recognized accrediting body (when working in a skilled, long term care facility).
  • Basic Life Support (BLS).
  • Depending on location, this role classifies as a Sanford Category I or II Driver requiring a motor vehicle report and proof of valid driver’s license.
  • Must be medically qualified to perform the essential driving functions of this position as per our Sanford Driving Policy.

Nice To Haves

  • Two years’ experience in this field preferred.
  • Knowledge of working with a wide variety of resident demographics.
  • Experience in social services, creative activities or long term care desired.
  • Successful completion of an accredited training program approved by state preferred.
  • Current Certified Nurse Assistant (CNA) certification or currently enrolled as a student in a healthcare related program preferred.

Responsibilities

  • Lead the planning and development of activity programming.

  • Act as a resource for employees, residents, and family with concerns around established activities and other matters related to activities.

  • Create and plan activities around established resident care plans.
  • Assess the needs of the audience and develop programs that meet the needs of the residents.
  • Complete activity assessments and minimum data set (MDS) questions as needed.
  • Strive to improve the quality of life for the residents by providing appropriate, meaningful activities and interventions on a one-on-one and/or group basis.
  • Administer the monthly activity calendar.
  • Elicit residents input into activity planning and keep activities well organized and upbeat, following all state required activities such as music, craft and spiritual activities.
  • Partake in activities such as fishing, nature walks, pet therapy, music therapy and others in order to support residents and ensure they are functioning at an optimal level while also maintaining dignity and identity.
  • Assist with simple nursing procedures and transporting residents to and from rooms.
  • Oversee the effectiveness of programming being implemented, and provide necessary feedback to enhance the quality of activities being implemented.
