About The Position

Apple Valley Health Care Center, a Cassia community, is hiring an Activities Assistant to join our team in our transitional care unit! At Apple Valley Village Health Care Center, you'll be part of a supportive, mission-driven team that takes pride in making a difference every day for residents and families alike. In this role, you will meet with new transitional care patients to conduct assessment interviews, provide activity resources, and help them have a good experience during their stay. Our ideal candidate is self-motivated with a genuine concern for our residents’ well-being. Must be organized, detail oriented, with meticulous documentation and communication skills, and have the ability to complete documentation accurately and within established timelines. This position requires a high volume of documentation for each interview completed.

Requirements

  • Demonstrate good time management and organizational skills.
  • Ability to display enthusiasm and a positive attitude in promoting activities.
  • A genuine curiosity and enjoyment meeting new people and listening to their stories.
  • Good communication skills, including ability to give directions to others in a manner that is pleasant and easily understood.

Nice To Haves

  • Prior experience working with seniors preferred, but not required.

Responsibilities

  • Welcome and interview newly admitted transitional care patients.
  • Learn about each patient’s interests, routines, family, career, hobbies, faith preferences, social connections and what is meaningful to them.
  • Identify activities, leisure interests and opportunities that may support each patient’s comfort, engagement, and quality of life during rehabilitation.
  • Encourage participation while respecting each patient’s individual preferences, abilities, and choices.
  • Complete required documentation including assessments, observations, care plans, and other assigned documentation.
  • Prioritize new admissions and documentation requirements in a busy transitional care environment.
  • Communicate relevant information and patient preferences with the interdisciplinary care team.
  • Assist with individual visits, small groups, and facility activities.
  • Document activity attendance and degree of involvement.
  • Act as a positive representative at all times.
  • Display an attitude of courtesy and respect for all residents, families, and staff.
  • Perform other related tasks as needed.
