About The Position

This position involves planning and conducting activities for residents, ensuring good communication between staff, residents, and families, and participating in community planning related to the facility's interests and resident needs. The role also includes assisting with the development of activity calendars, maintaining records, participating in discharge planning and care plan development, and arranging transportation for residents. Additionally, the Activities Assistant helps create monthly activity schedules, encourages resident participation in self-initiated activities, provides necessary materials, assists with documentation, and maintains the cleanliness and orderliness of the Activity Department.

Requirements

  • High school diploma or equivalent.
  • Ability to read technical procedures.
  • Ability to read and comprehend policy and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Standing and/or walking very frequently.
  • Reaching with hands and arms frequently.
  • Pushing/pulling very frequently.
  • Talking and/or hearing very frequently.
  • Tasting and/or smelling very frequently.
  • Lifting up to 50 pounds frequently.
  • Close, distance, color, peripheral, and depth perception in vision: ability to adjust focus.

Nice To Haves

  • One year experience in a long term care facility.
  • Climbing, balancing, stooping, kneeling, crouching or crawling occasionally.

Responsibilities

  • Plan and conduct individual, small, and large group activities.
  • Assist in maintaining good communication between employees, residents, families, support personnel, government agencies, and the public.
  • Participate in community planning related to the facility's interests and resident needs.
  • Assist in developing a monthly activity calendar and maintaining attendance records.
  • Assist the Quality Assurance and Assessment Committee in developing and implementing plans of action for quality deficiencies.
  • Participate in discharge planning, activity care plans, and resident assessments.
  • Assist in arranging transportation for residents for outings, appointments, or discharge.
  • Develop a monthly activity schedule for residents, including outings and in-room activities.
  • Encourage residents to participate in self-initiated activities.
  • Provide necessary materials, such as reading materials in Braille or audio books.
  • Assist with assessment documentation and quarterly progress notes.
  • Keep the Activity Department clean, orderly, and secured.
