This position represents an amazing opportunity for an ambitious APP leader ready to take on a crucial role building out and leading the Active Care Service for Devoted Medical Primary Care. As an Active Care APP Manager, you will be responsible for the clinical and operational oversight of a team of Advance Care Providers (APP) who will deliver world class primary care to Devoted Health Plan members. This position also includes direct clinical responsibilities in caring for our patients. On a day-to-day basis you will work closely with the primary care leadership team, the APPs on your team, and cross functionally to deliver on our ambitious objectives. This position offers the opportunity to impact patient outcomes at scale in a modern, tech-supported care environment. You will report directly to the Medical Director for primary care. Overview: This role will be APP Manager on the Active Care service. Active Care is one of the core teams within the primary care departments. The Active Care team is responsible for stabilizing and managing complex primary care patients with multiple chronic conditions. Active Care works with a panel of members with uncontrolled chronic disease, acute disease in need of investigation and treatment, and preventative health needs in a longitudinal care model alongside other APPs and Physicians. This role will launch in the Summer/Fall of 2026 and this role will serve as a crucial contributor to the buildout of the delivery model. This role is for a clinical leader who develops, supports and guides a team of APPs providing primary care through telehealth while also leading process improvement projects that help develop our primary care operation. The APP manager will excel at change management, communication, and culture building in a rapid change environment. The APP Manager will also consistently ensure their team delivers high-quality, member-centered care, adheres to workflows, meets program expectations and continues to grow clinically. This role requires strong communication skills, process improvement skills and a focus on performance coaching. You will also maintain a limited panel of visits to stay connected to clinical work and program workflows.
