About The Position

The ACT Team Program Manager provides daily leadership and administrative oversight of the ACT Team. This role involves supervising and evaluating clinical and non-clinical staff, ensuring fidelity to the ACT model, and coordinating treatment plans. The manager is responsible for monitoring client admissions, discharges, service delivery, and ensuring timely and accurate clinical documentation and billing. They will also maintain effective working relationships with patients and community partners, promote a trauma-informed approach, and ensure patients and families receive appropriate education. This position may require participation in an on-call rotation.

Requirements

• Daily leadership and administrative oversight of the ACT Team.
• Supervision and evaluation of clinical and non-clinical staff.
• Ensuring fidelity to the ACT model.
• Compliance with state, federal, Medicaid, and accreditation standards.
• Facilitation of daily ACT team meetings.
• Coordination of treatment plans.
• Monitoring client admissions, discharges, service delivery, and caseload assignments.
• Ensuring timely and accurate clinical documentation, billing, and reporting.
• Maintaining effective working relationships with patients and employees.
• Collaboration with external partners (hospitals, crisis services, law enforcement, housing providers, community partners).
• Monitoring program quality indicators, client outcomes, and performance improvement initiatives.
• Participation in on-call rotation.
• Promotion of a trauma-informed, recovery-oriented, culturally responsive approach.
• Ensuring patient and family education on treatment, services, and recovery.
• Maintenance of confidentiality and compliance with HIPAA and agency policies.

Responsibilities

• Ensures fidelity to the ACT model and compliance with state, federal, Medicaid, and accreditation standards.
• Facilitates daily ACT team meetings and coordinate treatment plans.
• Monitors client admissions, discharges, service delivery and caseload assignments.
• Ensures timely and accurate clinical documentation, billing and reporting in accordance with corporate, Medicaid, and regulatory requirements.
• Maintains effective working relationships with patients and other employees.
• Collaborates with hospitals, crisis services, law enforcement, housing providers, and community partners to coordinate care.
• Monitors program quality indicators, client outcomes, and performance improvement initiatives.
• Participates in on-call rotation as required.
• Promotes a trauma-informed, recovery-oriented, culturally responsive approach to service delivery.
• Ensures patients and families receive appropriate education regarding, treatment, services, and recovery to promote understanding and engagement.
• Maintains confidentiality and compliance with HIPAA and agency policies.
• Performs other duties as assigned.
