About The Position

Provide leadership, clinical supervision and oversight to case managers and other direct service staff as assigned. The ACT Team Leader will provide group and individual clinical/administrative supervision and leadership to assigned team members in the implementation and coordination of Health Home activities. This role ensures staff are trained in the development and implementation of client treatment plans that adhere to medical necessity guidelines and incorporate health home services. The leader monitors day-to-day activities, ensures productivity targets are met, provides orientation and ongoing training, and monitors clinical documentation for quality and timeliness. The position also involves providing direct clinical care, assisting in community networking, serving as a liaison to state behavioral healthcare programs, and assisting with recruitment and hiring. The role embraces trauma-informed principles and promotes effective working relationships.

Requirements

Master’s Degree in Behavioral Health, Human Services, or related field required.
Minimum of 10 years (10) behavioral healthcare experience.
4 years supervisory and managerial experience required.
Independent Clinical License required.
Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job.

Responsibilities

Provide group and individual clinical/administrative supervision and leadership to assigned team members in the implementation and coordination of Health Home activities.
Ensure staff is trained in the development and implementation of client treatment plans that adhere to medical necessity guidelines and incorporate health home services (Healthcare coordination, wellness and prevention, transitional care, individual and family support services, and comprehensive care management).
Monitor the day-to-day activities of assigned team members.
Work with Program Manager and Clinical Supervisors to ensure individual and team productivity targets are being maintained.
Provide orientation to new team members.
Provide ongoing trainings in prevention and health promotion activities, management of health conditions, and coordination with healthcare facilities and other community partners.
Monitor clinical documentation of assigned team members to ensure treatment plans, progress notes, and other client documents are in accordance with Thrive standards for quality, and submitted within established timeframes.
Provide direct clinical care in support of staff development and client needs.
Assist in community networking and education.
Provide direct clinical supervision in accordance with BHDDH Rules and Regulations.
Identify individual training needs for team members.
Monitor performance and staff growth or need for re-mediation.
Create and promote effective working relationships among team and colleagues.
Work collaboratively as part of a department/team to facilitate the program’s ability to meet its goals and objectives.
Maintain and facilitate a positive flow of communication within team, CSP program and Thrive as a whole.
Assist Program Manager in monitoring staff adherence to CSP and Thrive policies & procedures.
Be well versed in clinical care standards to ensure high quality service to clients.
Embrace trauma informed principles, guiding relationships with both staff and clients.
Serve as liaison to the state behavioral healthcare programs to insure facility licensing compliance to state contracts.
Assist Program Manager in recruiting and hiring of new staff.
Maintain responsibility for completing performance evaluations, including routine monitoring of staff performance and performance improvement plans, as indicated.
Attend all agency/department required training and meetings within the prescribed period.
Attend all Safety Training programs and be able to describe his/her responsibilities related to the general safety, department safety, and specific job-related hazards and responsibilities.
