About The Position

ACT Supervisor Engages individuals and families in a welcoming, hopeful, empathetic manner regardless of stage of readiness and phase of treatment. Conduct and attend daily team meetings to review problems and plan solutions, and completes meeting minutes on a rotation basis. Attends meetings with ACT psychiatrists at least twice per week to ensure continuity of individual care. Ensures that team members meet with individuals within 72 hours of a psychiatric admission to facilitate discharge planning. Assists individuals with medication compliance and distributes medication vials to individuals as ordered by the psychiatrist, as well as “eyes on meds”, to ensure compliance with treatment plan. Ensures and works within the rotation of ACT individuals to ensure that individual needs are met by all professional disciplines. Certify that paperwork is maintained in accordance with Medicaid and Department of Community Health guidelines, as well as Easterseals policies and procedures. Requirements

  • Hold licensure as a Licensed Master of Social Work (LMSW) or Licensed Professional Counselor (LPC) from the State of Michigan.
  • Must meet qualifications of a Child Mental Health Professional (CMHP) and/orMental Health Professional (MHP) per the Michigan PIHP/CMHSP Provider Qualifications per Medicaid Services.
  • Have had at least three (3) years of full-time casework, therapy, or assessment experience including 1 year in a leadership role.
  • Possess a valid motor vehicle operator or chauffeur's license.
  • Must possess knowledge and understanding of co-occurring disorder (mental health and substance abuse); stages of change (transtheoretical model); phases of recovery and motivational interviewing/enhancement therapy.
  • Knowledgeable with criteria for dual diagnosis capability for substance use and mental health programs.

Responsibilities

  • Engage individuals and families in a welcoming, empathetic, and recovery-oriented manner, regardless of readiness for treatment
  • Lead and participate in interdisciplinary team meetings to coordinate care, review cases, and support continuity of services
  • Support hospital discharge planning by ensuring timely follow-up and engagement after psychiatric admissions
  • Promote medication adherence through education, monitoring, and coordination with psychiatric providers
  • Oversee documentation and ensure compliance with Medicaid, state, and organizational standards
  • Maintain knowledge of ACT eligibility criteria and ensure appropriate service authorization
  • Provide clinical supervision, coaching, and training to staff, including onboarding and ongoing professional development
  • Facilitate staff participation in required trainings and ensure adherence to program standards
  • Conduct comprehensive assessments, including co-occurring mental health and substance use disorders, and support integrated treatment planning
  • Utilize Motivational Interviewing and strength-based approaches in supervision, team collaboration, and service delivery
  • Monitor outcomes and use data-informed practices to improve individual and program performance
  • Support quality improvement efforts, including performance management and corrective action planning as needed
  • Coordinate special projects and program initiatives as assigned
